MOUNTAIN SHADOWS COMMUNITY HOMES

 

 

POSITION: REGISTERED NURSE

REPORTS TO:  HEALTH SERVICES SUPERVISOR

 

BASIC FUNCTION: To monitor the on-going health status of residents in all assigned facilities and provide healthcare services which ensure the optimum health of each resident.

 

RESIDENT SERVICES:

 

* Participate in the admission/discharge procedure for all residents to including pre-                         admission evaluation, review of medical history and nursing care plan.

 

* Develop nursing care plan for any condition that requires more that 72 hours to resolve.                Discontinue care plan within 48 hrs of resolution.

 

* Review resident nursing care plans annually and semi-annually.    

 

* Complete and document resident weights by the 8th of each month.

 

* Complete and document resident height (until resident is 18 years old) and vital signs                   quarterly.

 

* Consult with physician on regular basis regarding resident health concerns.

   

Complete regular assessments of residents and respond to resident health concerns      expressed by parents, Direct Care Staff, QIDP, RC, day program, etc.                     

 

Confer with consultants regarding resident needs including dietary, physical therapy,                    psychiatric services, etc.

 

Complete annual sex education assessment.

 

            * Complete annual bowel and bladder assessment.

            

            * Participate in facility immunization clinics.

            

 * Complete annual TB testing via PPD skin tests, chest x-rays, or blood tests.

 

 

DOCUMENTATION AND CHARTING:

 

Complete/review GERs as necessary.

 

* Complete monthly and quarterly summaries by 15th of each month.

 

* Complete nursing assessments for annual and semi-annual IPP meetings utilizing THERAP          software program.

 

* Document and implement physician’s orders as necessary.

 

* Audit physician orders and medication administration record monthly.

 

 

* Complete follow up on quarterly pharmacy audit and correct any deficiency within 30 days.

 

            * Complete paperwork for medical/dental appointments.

 

* Follow up on medical/dental appointments and/or hospitalizations and chart as necessary in         resident record.

 

*Order lab work as ordered by the physician and chart as necessary in resident record.

  

GENERAL DUTIES:

 

* Complete weekly medication audits for each home.  Provide follow up as necessary such              as contacting physician or pharmacy. Document medication administration errors and                          provide discipline to staff if appropriate.

 

            * Supervise the training of staff in administration of medications and treatments.

 

* Audit monthly inventory reorder for house supply medications.

 

* Check cycle-fill medications monthly, including school medication.

 

* Provide training for staff, as necessary, on resident health issues.

 

* Assist with implementing MSCH safety program including, assessing staff injuries as             requested.

 

* Other duties as assigned.

 

MEETINGS AND COMMITTEES:

 

* Participate in Human Rights and Infection Control meetings quarterly.

 

* Participate in 30-day, semi-annual, annual and special/emergency Interdisciplinary Team              meetings.

 

 

* Attend Individual Education Plan (IEP) when requested.

 

* Attend RN meetings.

 

* Attend house meetings monthly

 

* Participate in Health Services and MSCH Safety Committee meetings, as required.

 

KNOWLEDGE AND ABILITIES:

 

KNOWLEDGE:

 

* ICF-DD/H and ICF-DD/N regulations

* DD population

 

ABILITIES:

 

* Effective verbal and written communication skills.

* Ability to use a computer and learn computer software programs.

* Maintain records and prepare reports.

* Complete necessary tasks promptly with multiple interruptions.

* Meet the required deadlines.

* Good interpersonal communication skills.

* Learn and interpret rules, regulations and policies.

 

EDUCATION AND EXPERIENCE:

 

Current and valid California RN license. Previous experience with persons with developmental disabilities preferred.  Able to obtain criminal clearance through the Department of Justice.

 

WORK SCHEDULE:

  • Full-time
  • Monday through Friday, 8-hour shifts, Flexible hours and rotating on call weekends

 

SALARY RANGE: 

  • $50-54/hr
