We are looking to hire a full-time Registered Nurse Supervisor to lead our QCN Home Health Care Medicare nursing team at our corporate offices in Troy, MI. This person will report to the Director of Nursing and will be responsible for the day-to-day care of our external patient base while working closely and communicating with our internal and coordinating care teams.

Essential Job Duties and Responsibilities:
- Conducting OASIS-D patient health assessments
- Developing and reviewing nursing care plans that are individual to each patient per Medicare guidelines
- Providing care according to the care plan ensuring that the emotional, physical, comfort and safety of patients are met to the greatest possible extent
- Performing routine nursing duties such as providing care to patients
- Observing and documenting patients’ conditions in software system (Alayacare)
- Understanding and implementing infection prevention practices
- Notifying the Director of Nursing of any health and safety risks or concerns
- Maintaining confidentiality of client and corporate information
- Responsible for compliance and reporting requirements
- Completing other tasks as requested, such as occasional backup of traditional homecare leadership
Requirements
- Active RN License in the State of Michigan
- Previous Medicare home care experience (3-5 years)
- Able to start intravenous lines, training can be provided
- Synergy EMR experience
- OASIS documentation experience
- Case Management experience preferred but not required
- Work location varies between Troy office and patient homes, as needed
- Reliable transportation required
- Able to use medical software in the home via tablet and/or smart phone
- Vaccination verification of COVID-19 (preferred)
This is a full-time, 8:30 am - 5:00 pm, Monday - Friday salary position; salary is commensurate with experience.
