Licensed Practical Nurse

Position Summary:

The Licensed Practical Nurse is responsible for assisting providers with office visits and provide skilled nursing services as scheduled and document information in the organizations EMR. This position will play a vital role in patient quality care.

 

Job Responsibilities: 

  • Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards and Maryland State Regulations. 
  • Using the electronic medical records (EMR), input data and following through seeing services to their completion. 
  • Take part in all agency training programs as requested.
  • Complete specialty referrals on behalf of providers: collaborate with specialty offices to ensure continuity of care and receipt of specialty services.
  • Complete prior authorizations for services and medications; collaborate with pharmacy, pharmaceutical, and service vendors to ensure continuity of care. 
  • Administer prescribed medications, including injections under the supervision of the prescriber or register nurse.
  • Collect and process lab specimens.
  • Complete all regulatory documentation of client treatment and share information in a timely manner with other professionals working with client, both with the staff and, with client permission, outside agencies.
  • Assist providers with office visits and provide skilled nursing services as scheduled and document information in the organizations EMR.
  • Escort patients to exam rooms, interviews patients, measure, and document vital signs.
  • Monitor the nurse line to triage and respond to client needs through collaboration with the care team, which includes pharmacy coordination.
  • Keep exam rooms stocked with adequate medical supplies, maintain instruments, prepare sterilization as required; Properly disposes of contaminated supplies 
  • Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls
  •  Collaborate with rehabilitation/treatment plans and staff in serving the client’s needs.
  • Provide health promotion and training as requested.
  • Performs other duties as assigned by supervisor.

 

 

Qualifications, Educations and Experience: 

  • Current, active Licensed Practical Nurse in the State of Maryland
  • Two years of nursing experience, preferably working with those who have mental illness.
  • Demonstrated ability to work collaboratively in a teamwork environment.
  • Highly developed organizational and communication skills.
  • Availability for work including some evenings as needed.
  • Current CPR certification.
  • Privileged to carry out all duties, including client treatment coordination.
