What You’ll Be Doing

As an NHV coordinator you'll be playing a direct role in managing services for the individuals we serve.  Acting as liaison among person served, their families/advocates, PCA's, healthcare providers, and NHV's scheduling and administrative team.


Role & Responsibilities:

  • Person-centered Planning: Each individual has a support plan tailored to their preferences and needs.  You will work with family/advocates, regional center, and the individual to develop and review Individual Program Plans (IPP) and person-centered goals.   
  • Supervising and coaching PCAs: Ensuring that Personal Care Attendants understand their responsibilities and follow NHV & state policies. 
  •  Medication Management:  This includes arranging prescription pick-ups, tracking medications, maintaining the Medication Administration Record (MAR), and instructing PCAs about proper medication administration. 
  •  Health & Safety Oversight:  Monitor client health, respond to concerns, and arrange appropriate follow-up with healthcare providers. 
  • Administrative tasks: Complete reports, maintain documentation, lead house meetings, and participate in training or      quality-assurance activities.
  • Attending / Leading Team Events

Requirements

 What You Bring to the Table:

  •  A proactive and positive attitude with a willingness to learn and adapt.
  •  Great communication skills, even when communication is challenging.
  •  An ability to maintain professionalism with person served, colleagues, and willingness to work cooperatively in a team. 
  •  First Aid and CPR certification (or willingness to get certified by us). 

Qualifying Specifications:

  • Must have own vehicle, maintain a current California's Driver License and provide on-going proof of insurance requirements as required by State and New Heights Ventures.  Must have/maintain acceptable driving record.
  • Ideal candidates will reside in San Luis Obispo City or Northern SLO County (Paso, Templeton, Atascadero etc.)  We understand that many enjoy a commute, however, for response time/on-call, location makes a huge impact.
  • Must clear fingerprints through the California Department of Justice & FBI
  •  Legible writing skills, in English, for accurate documentation.
  •  Prior experience
