Job Description


Job Title: PrEP Case Manager 
Department/Division: HIV Medicine
Reports to:  Client Service Manager 

Education: Bachelor’s degree in social work, Human Services, Psychology, or related fields required.  One to three years documented experience in case management services.  Working knowledge in the field of HIV and STI’s, familiarity with community resources, the Harm Reduction model, ability to work in a fast paced, interdisciplinary team setting strongly preferred. 
Position Function:  The PrEP Case Manager is responsible for  providing high-quality, client-centered case management services to patients in the PrEP (Pre-Exposure Prophylaxis) program. The Case Manager will conduct screenings and intakes to determine program eligibility, in addition to developing individualized care plans. In addition, the PrEP Case Manager will provide HIV and STI risk reduction strategies, as well as   medication adherence assistance.  The PrEP Case Manager will have extensive client interaction and will collaborate with community partners in recruitment efforts to identify patients who are appropriate for PrEP services. 

Job Duties and Responsibilities:
•    Conduct thorough screenings and intakes to assess clients' readiness for PrEP/PEP. Identify potential barriers to adherence, such as financial, housing, mental health, or substance use issues.
     Develop and implement personalized care plans that address each client's unique needs, goals, and challenges.
•    Understand and facilitate the Pre-Exposure Prophylaxis for HIV Prevention policies and procedures as designated in the Division of HIV Medicine Policy and Procedures.
•    Complete training to conduct rapid HIV testing and counseling (?). 
•    Expand AMC’s PrEP print and online educational materials to ensure the collection, collation and organization of resources, pamphlets, electronic articles and reference information.
•    Participate in community outreach events/sessions for priority populations to educate and promote PrEP program and link them to services.
•    Provide education and training to community organizations and AMC departments regarding PrEP implementation to increase prescribing among providers.
•    Provide comprehensive education on PrEP/PEP, including adherence strategies, benefits, and potential risks. Offer counseling on risk reduction and healthy lifestyle choices.
•    Provide ongoing follow-up with clients to monitor PrEP adherence, schedule medical visits, and assess progress towards their care plan goals.
•    Determine patient eligibility for Medicaid, PrEP-AP, Gilead Advancing Access program and/or any other program that could prevent individual financial barriers to PrEP treatment.
•    Maintain accurate and confidential client records in compliance with HIPAA and other standards. Submit regular reports on client outcomes, program effectiveness, and other relevant data. Ensure proper documentation, collection, and data entry used in reporting, research, and program evaluation i.e.: EPIC, AIRS, OAS.
•    Assists patients in developing self-management and health literacy skills.
•    Use Motivational Interviewing to impact behavior change related to an individual’s risk of HIV/STI acquisition, medication adherence or other issues.
•    Understand legal issues around Domestic Violence and abuse issues and intervenes appropriately and with accordance of Albany Medical Center’s Policy and NYS Law. 
•    Work closely with the Community Advisory Board (CAB) to revise, assist, and communicate changes and recommendations for PrEP services.
Additional Responsibilities
•    Develop collaborative relationships with community agencies to facilitate PrEP services for negative individuals and for their newly diagnosed clients to get connected to care.
•    Provides occasional cross coverage of other case management functions, as needed.
•    Other duties as assigned by the manager of client services or designee.
•    Updates release of information forms, health care proxy, and other pertinent information annually.

COMMUNICATIONS SKILLS:
•    Evaluates patient circumstances and communicates with the patient in an objective, nonjudgmental manner.
•    Strong critical thinking and problem-solving abilities to address client barriers.
•      Cultural competency and empathy to build rapport with a diverse client base.
•    Addresses interpersonal conflicts in a direct and professional manner.
•    Responds promptly to email and phone messages; responds to routine inquiries or complaints from patients, regulatory agencies, or members of the community.
•    Knowledge of medical terminology  
•    Fluency in written and oral English using the language in a proper and professional manner.
•    Fluency in Spanish is a plus.

COMPUTER SKILLS:
•    Typing 
•    Basic PC skills, including navigation of hard drive, share drives/networks, internet, and email systems, accessing files and saving files to a variety of locations.
•    Fluency in Microsoft Word
•    Ability to use Microsoft Excel documents.
•    Ability to learn computer programs for scheduling, data entry, and medical record documentation.

MATHEMATICAL SKILLS:
Ability to calculate figures and amounts such as discounts, interest, proportions, and percentages.  Ability to develop a budget.

REASONING ABILITY:
Ability to define problems, collect data, establish facts, and draw valid conclusions.
