About the Department
Provides professional and technical assessment, case management and direct service and assistance to clients in various Human Services Programs.
Position Duties
- Provides case management services to assist clients in receiving assistance, care or treatment, counseling or related services through a social services program.
- Makes site visits and assesses the quality of care and compliance with program requirements.
- Conducts interviews and assessments to identify individual client needs and/or risks.
- Testifies in court as necessary.
- Determines, monitors and maintains program eligibility and administers
- services within local, state or federal guidelines.
- Provides crisis intervention and counseling to assist clients in resolving issues and other barriers to obtaining services.
- Develops action or treatment plans, makes referrals and coordinates services to meet individual client needs; makes recommendations for short and long-term treatment, care or assistance.
- Documents action/treatment plans, client progress, and services provided.
- Data entry, retrieval and maintenance of records, files and other materials according to established methods and procedures.
- Determines need for recovery of funds or referral for fraud investigation.
- Acts as an advocate for the client; coordinates with collateral agencies, families or service providers; provides follow-up case management to assist clients in resolving problems in receiving quality care and/or services.
- Responds to requests for information and act as technical and referral source for clients and public regarding particular program assignments.
- Provides administrative support for assigned program; compiles program statistics and coordinate the processing of appropriate paperwork.
- Acts as a resource or expert in program area, providing technical guidance to less experienced staff.
- Attends seminars, conferences, workshops, classes, lectures, etc., as appropriate, to enhance and maintain knowledge of trends and developments in field; review professional journals, attend association and professional meetings; and otherwise maintain contacts with professionals to facilitate exchange of information.
- Compiles data and information and completes various forms and reports.
- Performs other duties as appropriate or necessary for performance of the job.
- Engages clients in the development of Individual Responsibility Contracts, employability plans and/or other agreements that outline agency and client responsibilities.
- Monitors, tracks and supports progress as clients’ moves toward their goals.
- Understands family dynamics and recognizes potential barriers to employment such as domestic violence, drug and alcohol abuse, mental health or any other situations requiring immediate intervention and refers as appropriate.
- Outcomes are tied to clients participating in specified federal work activities for a determined number of work hours per month.
- Determines supportive services needed to fulfill the service plan.
- Works with community partners, employment counselors and other agencies to maximize clients’ success
- Conducts client home visits for purpose of crisis intervention, monitoring, needs assessment and at-home assistance applications.
- Conducts interviews and reviews applications submitted by clients to gather information required to determine eligibility for Colorado Works, Medicaid and Food Assistance Programs.
Minimum Qualifications
EDUCATION:
- High School Diploma or GED equivalency. Required
EXPERIENCE:
- Minimum of one year of full-time professional case management and/or social casework experience required in a public or private social services agency in a service delivery assignment appropriate to the position. Required
- Minimum of three years experience as a Technician within the Department of Human Services, Required
- Successful completion of twenty hours of training in a human behavioral area that enhances the ability to perform the essential functions of the job required.
PERFERRED QUALIFICATIONS:
- Bachelor’s Degree from an accredited four-year college or university in one of the human behavioral science fields such as human services, social work or psychology. Preferred
LICENCES and/or CERTIFICATIONS:
Valid driver’s license at the time of application and at time of application and a valid Colorado driver's license at time of hire. Required
