Position Summary:

Regional Case Manager to support the case management and managed care intake functions for a number of client skilled nursing centers in the Mountain Time Zone.

 

Full-time benefits start on the first day of the month following your start date!

Posted Salary Range:USD $75,000.00 - USD $85,000.00 /Yr.Wage Commensuration:The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs. Duties & Responsibilities:
  • Assesses, plans, implements, coordinates, and monitors patient transitions along the entire SNF continuum of care from authorization to discharge from SNF.
  • Coordinates service delivery; working with interdisciplinary team to meet the client’s individual healthcare needs, ensuring smooth transitions for all.
  • Coordinates care utilizing referral sources requirements, establishes working relationships with referral/payor sources and facilities.
Qualifications & Requirements:
  • Ideal candidate must be a skilled written/verbal communicator, team player with excellent organizational skills. Able to multitask effectively and be very detail oriented.
  • 3-5 years licensed clinical or nursing experience with primary focus in SNF/long term care and involved with Case Management/Managed Care process.
  • MUST have Case Management experience to be considered.
  • Must have valid Nursing or other healthcare license in Texas. Individuals holding other healthcare licensure will be considered on a case by case basis.
  • Must have strong computer skills. Familiarity with Microsoft Office and strong Excel skills required.
