What You'll Do:
The RN Case Manager is a Registered Nurse who provides clinical case management services to individuals experiencing a behavioral health crisis. This role is responsible for facilitating psychosocial assessments, stabilization, treatment planning, and discharge planning. The RN Case Manager works collaboratively with the interdisciplinary team, individuals, families, and community providers to ensure safe, effective, and person-centered care.
- Conducts a thorough, accurate, clinically sound psychosocial assessments with individuals in crisis.
- Participates in the development, implementation, and review of individualized, person-centered treatment plans in collaboration with the interdisciplinary team, the individual, and their support system.
- Gather collateral from the individual and involved parties to provide a thorough assessment of what an individual's needs are for treatment and discharge.
- Facilitates admission intakes, discharge planning, and follow-up care to ensure smooth transitions and continuity of services.
- Coordinates care and referrals with internal staff, families, clinics, local providers, and community resources.
- Maintains knowledge of the Involuntary Treatment Act (ITA) and related processes, monitors petitions, and participates in hearings when required.
- Maintain the Electronic Health Record (EHR), including documentation of activities performed as part of the service delivery process. Obtains signatures from individuals regarding treatment, as necessary. Document all services and individuals’ activities in the Electronic Health Record (EHR) regarding brief interventions, coordination of care, discharge planning, treatment planning, and any other pertinent information.
- Prepares clinical reports and individualized service plans utilizing agency electronic clinical documentation software. Ensures documentation complies with best clinical practices, contracts, regulatory, and other legal requirements.
- Maintains a working knowledge of the Washington State behavioral health system and potential funding sources.
- Participates in treatment team meetings and ensures timely, accurate, and compliant documentation of all clinical services in the Electronic Health Record (EHR).
- Demonstrates competency through ongoing professional development, supervision, and participation in required trainings.
- Performs all other duties as assigned.
What You'll Bring:
- Must meet Medicaid State Plan definition of Psychiatric Nurse:
- A Registered Nurse (RN) with a bachelor's degree from an accredited college or university, with at least two years of direct experience in the treatment of mentally ill or emotionally disturbed persons under the supervision of a Mental Health Professional; OR
- A Registered Nurse (RN) with at least three years of supervised experience. Three years of experience in mental health, substance abuse treatment, or relevant work.
- Must be able to perform Safe Clinch patient restraint techniques.
- The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment
- Complete all credentialing requirements within the required time period, including but not limited to obtaining a National Provider Identifier (NPI), Medallion enrollment, Medicare enrollment, and any other necessary credentials as required for the role
It would be great if you had:
- Master's degree in Nursing (MSN) from an accredited college or university, with a specialization in psychiatric/mental health nursing preferred
- Experience working with children, youth, families, and adults with behavioral health issues
- Knowledge of and experience with the Washington State involuntary treatment process

The typical hiring range for this role in Washington is $40.50 - $47.75 per hour.
