The Youth Services Coordinator involves a combination of social work, advocacy, and support services for young adults aging out of the foster care system.
Essential Duties:
- Assessment and Planning: Conduct assessments to determine the specific needs and goals of client. Develop individualized service plans to address their unique circumstances.
- Life Skills Development: Provide training and assistance in developing essential life skills, such as budgeting, cooking, job searching, and apartment hunting.
- Education and Employment: Assist in accessing educational opportunities, such as GED programs or college enrollment, and facilitate job readiness training and job placement services.
- Housing Assistance: Help secure safe and stable housing, which may include assistance with finding affordable apartments, understanding leases, and accessing housing subsidies or vouchers.
- Financial Literacy: Educate clients about financial management, including budgeting, saving, and establishing credit. Connect them with resources for financial assistance if necessary.
- Healthcare Access: Assisting with access to healthcare services, including Medicaid or other health insurance programs. Assist with making medical appointments and understanding healthcare options.
- Mental Health and Counseling: Provide emotional support and referrals for mental health counseling or therapy as needed.
- Legal Support: Help with legal matters, including navigating the legal system, understanding their rights, and obtaining legal documents such as birth certificates, social security cards, and identification.
- Advocacy: Advocate on behalf of clients when necessary, such as during interactions with government agencies, educational institutions, or potential employers. Ensure their rights are upheld.
- Crisis Intervention: Be available for crisis intervention and support during challenging times. Assist in finding emergency shelter or resources in critical situations.
- Monitoring and Follow-Up: Continuously monitor the progress of clients and make adjustments to their service plans as needed. Maintain regular contact to provide ongoing support.
- Documentation: Maintain detailed records of interactions, assessments, and progress made by each client.
- Community Resources: Connect clients to community resources, such as childcare services, food assistance programs, and legal aid, to address their immediate needs.
- Transportation: Assists with transportation to school and various appointments.
Requirements
- Bachelors degree in field of social services preferred or 5 years working with youth.
- Competency in Microsoft Office for Windows applications (word processing, database, spreadsheet, e-mail, presentation graphics, etc.) is required.
- Proficiency in general office equipment operation is required.
- Must possess exceptional time management and detail-orientation skills.
- Must be at least 21 years of age.
- Must possess a valid Texas driver’s license.
- Must be available for travel as needed.
- Available to respond and intervene in family crisis situations.
