External Care Manager - ECM
This position is responsible for overseeing caregivers in the field with their client and ensuring care is being provided in a professional, caring and safe manner. This position will work with their client, family and staff to develop a careplan and task list for staff working with each client. They will maintain a client load and oversee client satisfaction with the caregivers working in the home, our services and overall agency experience.
Additional Duties
- Observes proper conduct while in resident’s homes.
- Adheres to Agency’s infection control and safety policies.
- Attends Agency’s in-service programs and staff meetings as needed.
- Assists in orientation of new personnel as requested.
- Respects client’s beliefs, cultures and limitations without prejudice.
- Practices effective cost-containment and accountability with supplies and equipment.
- Maintain an on-going audit of clients assigned to ECM
Experience Required
- Minimum of 2 years’ experience in a health care setting, preferably home care.
- Self-directed, independent worker.
- Ability to communicate effectively orally and in written form with residents, families, staff, vendors and the general public.
- Must have compassion and desire to work with the elderly.
- Must meet all health requirements including TB and background check.
- Maintains necessary licensure and certifications as required.
- Prefer Medical Assistant or Certified Nursing Assistant; or equivalent experience
