As a Social Worker at Monadnock Community Hospital, you will play a pivotal role in our mission to improve the health and well-being of our community. Your primary responsibility will be to provide compassionate and comprehensive social work services to patients and their families, ensuring they receive the support they need during their healthcare journey.
The Social Worker’s (MSW) role is to work within the Collaborative Care Model (CoCM) in conjunction with the Primary Care Physician (PCP), Psychiatric Mental Health Nurse Practitioner (PMHNP) and the multi-disciplinary team to identify, target treatment, perform triage, and manage the behavioral health needs of patients within the primary care setting. The MSW is a professional functioning within the scope of their training, in accordance the National Association of Social Worker’s Code of Ethics as well as MCH policies and procedures.
Key Responsibilities:
Patient Assessment: Conduct thorough assessments to identify the psychosocial needs of patients and their families. Collaborate with the healthcare team to develop care plans that address these needs, including referral development. The MSW will assist PCP’s to manage patients who use medical visits to obtain needed social support. The MSW will efficiently move patients into the appropriate behavioral health specialty care, when indicated.
Counseling and Support: The MSW position helps to decrease unnecessary use of the Emergency Department.
The MSW will work to improve the health status of the MCH population through increasing adherence with life-style change regimens, e.g., smoking cessation, weight management, etc.
Follows up with in person, telehealth, or telephone appointments where necessary.
Assists in in-patient consultation for behavioral health related issues and discharge if necessary. Providing resources for patient continuation of care.
Collaboration: Assists primary care health providers in the recognition and treatment of mental disorders and psychosocial problems.
Aids in the early detection of “at risk” patients, with the aim of preventing further psychological or physical deterioration.
Assists health care providers in preventing relapse or morbidity in conditions that tend to recur over time.
Crisis Intervention: Provides crisis intervention and emotional support to patients and families facing difficult medical decisions, end-of-life care, or other challenging situations.
Resource Coordination: Connects patients and families with community resources, such as counseling, support groups, financial assistance, and home health services, to enhance their overall well-being.
Advocacy: Serves as an advocate for patients, ensuring their voices are heard and their rights are respected within the healthcare system.
Documentation: Maintains accurate and confidential records of all interactions and interventions in compliance with hospital policies and legal regulations. Records patient’s medical record and updates medical history as necessary.
Completes Social Determinants of Health (SDoH) screenings.
Collaboration: Works collaboratively with healthcare professionals, including doctors, nurses, and therapists, to provide holistic patient care.
Education: Assists patients by preventing and managing addiction to pain medicine or other controlled substances.
Assists in the prevention and management of functional disabilities.
Aids healthcare providers in obtaining quality clinical outcomes with a high prevalence of mental disorders.
Helps PCP’s treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
Over time, the MSW seeks to improve the quality of PCP interventions. Assists in team building within practice.
