About the Role:
The Case Facilitator I - Child and Adolescent plays a critical role in supporting the health and well-being of children and adolescents within an outpatient healthcare setting. This position focuses on coordinating care plans, facilitating communication among healthcare providers, families, and community resources to ensure comprehensive and continuous care. The role requires a compassionate and organized professional who can assess client needs, monitor progress, and advocate for appropriate services to promote positive health outcomes. The Case Facilitator will work closely with multidisciplinary teams to implement individualized care strategies tailored to each child's unique circumstances. Ultimately, this position aims to enhance the quality of life for young clients by ensuring they receive timely, effective, and coordinated healthcare services.
Minimum Qualifications:
- High school diploma or equivalent required; Associate’s degree in social work, psychology, or related field preferred.
- Experience working with children and adolescents in a healthcare or social service setting.
- Basic knowledge of child and adolescent development and common behavioral health issues.
- Strong organizational and communication skills.
- Ability to maintain confidentiality and adhere to ethical standards in healthcare.
Preferred Qualifications:
- Bachelor’s degree in social work, psychology, counseling, or a related human services field.
- Experience with case management or care coordination in an outpatient healthcare environment.
- Familiarity with community resources and social service systems relevant to children and families.
- Training or certification in trauma-informed care or behavioral health interventions.
- Proficiency with electronic health records (EHR) and case management software.
Responsibilities:
- Conduct initial assessments and gather relevant information to understand the needs of children and adolescents.
- Develop, implement, and monitor individualized care plans in collaboration with healthcare providers, families, and community agencies.
- Facilitate communication and coordination among multidisciplinary teams to ensure seamless service delivery.
- Provide ongoing support and advocacy for clients and their families to access necessary healthcare and social services.
- Maintain accurate and up-to-date documentation of case activities, client progress, and service outcomes.
- Identify barriers to care and work proactively to resolve issues that may impact client engagement or treatment adherence.
- Participate in team meetings, case conferences, and training sessions to stay informed of best practices and program updates.
Skills:

Education: High School diploma or equivalent.
Experience: Must meet OBHP requirements = Two years' experience in mental health or community services preferred.
Must hold a state issued driver's license.
First shift, Full time, Monday-Friday, No holidays or weekends.
