Lead Clinical Social Worker

Location: Holy Cross Health
Department: Care Management
Schedule: Full-Time

Position Purpose

Holy Cross Health is seeking a compassionate and experienced Lead Clinical Social Worker to provide supervisory support to the Care Management Department and lead social work programs across Maternal Child Health (MCH) services—including the clinic, labor and delivery, high-risk pregnancy unit, postpartum unit, NICU, and pediatric unit. This role ensures compliance with Maryland statutes and social work practice guidelines while supporting patient safety and the mission of Trinity Health and Holy Cross Health.

What You Will Do

  • Supervise social workers within the Care Management Department and oversee MCH social work programs.
  • Coordinate and direct discharge planning, social services, and care coordination across the hospital and with external agencies.
  • Provide complex discharge planning, psychosocial assessments, and supportive counseling for patients and families.
  • Refer patients to appropriate community agencies and resources.
  • Assist hospital staff with reporting suspected abuse, neglect, or exploitation of vulnerable populations.
  • Implement, review, and revise social work policies and practices.
  • Collaborate with interdisciplinary teams to ensure patient-centered, effective, and efficient care delivery.

Minimum Qualifications

  • Education:
    • Master’s Degree in Social Work required.
  • Licensure/Certification:
    • State of Maryland LGSW, LCSW, or LCSW-C required.
    • Certified clinical supervisor in Maryland preferred.
  • Experience:
    • Minimum of 3 years of hospital or protective services social work experience.
    • Minimum of 3 years of Maternal Child Health social work experience.
    • Experience as a registered supervisor providing oversight to licensed social workers.
  • Skills:
    • Ability to work with patients across all age groups.
    • Strong communication skills with good audio and visual acuity.
    • Effective time management and task prioritization.
    • Knowledge of community resources and care coordination across the continuum.

Pay rate: $35.41 - $53.12

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
