Make a difference in the lives of New York City's residents and communities. For over 60 years PSS has performed vital and important work -- through its ten centers and affiliated programs. The agency touches the lives of over 2,000 people each day. As a Per Diem Social Worker, you will be responsible for coordinating supportive services for community members in one of the many communities that PSS serves. The Social Worker’s role is vital in ensuring effective supportive services are put in place that allows the residents to remain independent, active and to thrive in their home and community. 



This is a part-time non-exempt contract position that reports directly to the Director of Community Social Services. 



RESPONSIBILITIES:

  • Provide advocacy, support, and general case assistance services, including the conducting of intakes and making referrals, to all members of the center. 
  • Collaborate with other agencies and service providers in the local community, to best support center members and access resources. 
  • Collaborate with center staff to strategize for how best to coordinate support services for members and schedule new intakes. 
  • Educate center members on benefits, application procedures, clients’ rights, and provide advocacy and follow up when appropriate. 
  • Document engagements with community members in detailed case notes within the Peerplace database. 
  • Report accurately and timely the units entered into Peerplace, as well as entering units for Telephone Reassurance and Friendly Visiting. 
  • Coordinate with the Silver Star Worker(s) to ensure smooth reporting and documentation of services provided. 
  • Help community members build informal support networks with other neighborhood residents, families, and friends. 
  • Promote the agency, as well as Telephone Reassurance and Friendly Visiting services by performing outreach, participating in community events, and engaging community members in the neighborhood. 
  • Communicate with supervisor and other staff in a timely fashion and meet all required deadlines. 
  • Actively pursue and take full advantage of opportunities for professional growth and development.  
  • Comply with PSS personnel policy guidelines. 
  • Attend Supervision, Team, and Department Meetings. 
  • Perform additional duties as assigned by supervisor. 


QUALIFICATIONS AND EXPERIENCE:

  • Bachelor’s Degree in Social Work
    (Master’s preferred) 
  • Strong Program Management Skills 
  • Experience working with older adults
  • Superior Interpersonal and Organizational Skills 
  • Superior communication skills (written and oral) 
  • Bilingual fluency (English/Spanish) highly preferred
