As a Case Manager with a housing focus through the 1115 Waiver Program, you’ll take on the exciting and meaningful role of Housing Advocate for Catholic Charities of Delaware, Otsego, & Schoharie Counties. You'll be the go-to support for individuals facing housing challenges—helping them find stability, connect with vital resources, and take steps toward brighter, more independent futures.

- Use community resources to assist residents in meeting their financial, medical, employment, or educational needs.
- Coordinate and act as a liaison between community agencies and the residents.
- Ensure all resident benefits are activated prior to admission and remain active for the duration of their affiliation with Catholic Charities.
- Obtain resident food stamp benefits monthly for the agency.
- Assist residents with all paperwork relating to benefits and attend any re-certifications that are scheduled.
Case Manager Minimum Salary: $19.00 / Hour
Case Manager Maximum Salary: $20.00 / Hour
Case Manager Schedule: Monday – Friday 9am - 5:30pm (40 hours a week)
Qualifications:
Associates Degree in Human Services or related field
1-3 years of experience in mental health, substance use, or human services
