About the Department

The NCSB is seeking a Case Manager IV in their Housing and Homeless division. The Housing and Homeless division serves individuals experiencing homelessness in Norfolk through a variety of programs including outreach, housing planning, emergency shelter, and supportive housing programs.
The Case Manager IV will provide comprehensive services to persons exiting Eastern State Hospital into the community. Provides relationship development for landlord recruitment. Provides assistance to consumers in the housing application process and for training and support to ensure housing stabilization. NCSB Staff roles may include assisting other units at times to assist them with coverage needs, specialized assistance, or to cross-train. Staff hired into direct service positions (direct service, monitoring direct service, supervising direct service) can work in units other than their primary role to provide administrative or direct service activities within their skill set, education, and training. They may also serve in "voluntary" activities outside of the NCSB's core services working with the public for community outreach events, service fairs, homeless outreach activities, homeless day shelters, neighborhood outreach events, trainings, etc. in a direct service, clinical, or administrative role.
Position Duties
- Active engagement of persons that are identified as ready be exited from the state hospital. Active engagement of persons residing in the community, follow-up for effective engagement, and follow-through with making connections to housing and stabilizing existing housing.
- Provide unit training to new staff in the form of shadowing.
- Keep and submit accurate documentation of activities. including the collection and documentation of required information.
- Provide activities to overcome barriers to successful housing attainment.
- Assist with the development of applications for housing programs.
- Provide services in a person-centered environment, utilizing best practices and methods as directed by the grant activities.
- Work in the field and in the office, plus providing services in community settings such as streets,
public spaces, and benefits offices. Provides follow-along services to persons once housed to effectively transition them to those providing ongoing supports. - Provide development of a housing acquisition plan based on the person's needs and preferences. Work effectively with rest of team to ensure ongoing work with consumer on the plan.
- Act as consumer advocate to ensure their voice is heard and barriers are overcome to meet critical and preference needs.
- Provide supervision to unit Interns.
- Provide direct assistance including attending appointments with consumers.
- Provide crisis counseling and intervention when necessary.
Minimum Qualifications
Considerable experience (3 years) in a human services field related to the area of assignment. Some assignments may also require specific experience working with various types of client populations; master's degree preferred in some areas.
Preferred:
- Experience working with people experiencing homelessness and behavioral health disorders.
- Experience assisting people in gaining housing.
Other Qualifications
- Valid Driver's License may be required depending on assignment.
- Certification in basic first aid and CPR, depending on area of assignment.
- QMHP-A preferred.
