The RN Weekend Clinical Supervisor assumes overall responsibility for the implementation, delivery, coordination and supervision of hospice services provided during weekends. This position supports established goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.
RESPONSIBILITIES:
- Assumes duties of Administrator on Call for the weekends
- Assumes overall responsibility for oversight and management of the clinical services provided by the interdisciplinary team (IDT) within the homecare and facility setting during work shift hours
- Receives case referrals, ensuring new patient referrals are processed and scheduled; reviews available patient information related to case, including home visits, to determine hospice/home care needs; assigns appropriate hospice personnel to case as needed; conferences with Medical Director regarding any questions about an individual's eligibility for services
- Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate
- Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing Plan of Care
- Provides weekend summary report to Clinical Leadership team
- Serves as a backup to weekend team for patient visits as needed
- Complies with accepted professional standards and principles
- Collaborates with leadership team in strategic planning activities, identifying opportunities to improve services and better meet client needs
- Collaborates with operations and clinical staff to ensure the intake process meets the needs of the patient, family and physician/other referral source
- Participates in orientation, training, on-going education programs, instructing and coaching to ensure hospice personnel understands and can clearly articulate the hospice philosophy from intake through bereavement
- Other duties as directed
Requirements
- Current license to practice as a registered professional nurse in Georgia. Bachelor’s or Master’s degree in Nursing or related human service field.
- Certified Hospice and Palliative Care Nurse (CHPN) preferred
- Current CPR certification
- Minimum of five years of hospice experience
- Minimum of three years demonstrated ability to successfully manage and supervise clinical and administrative staff
- Excellent observation, verbal and written communication with a strong problem-solving skill set
- Belief in the hospice concept
- Valid driver’s license. Reliable, insured, automobile for work travel.
