Licensed Social Worker

About Us
At Legacy Hospice, we�re committed to caring for patients and families with compassion and dignity. We are seeking a skilled and empathetic Social Worker (SW) to join our team. In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis�bringing comfort and clarity during their most vulnerable moments.
What You'll Do
- Conduct psychosocial assessments of patients and caregivers
- Provide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning
- Assist with advance directives, funeral arrangements, and navigating legal/financial decisions
- Identify and coordinate with community resources, agencies, and support organizations
- Educate staff and families about cultural and psychosocial needs
- Document patient conditions, interventions, and psychosocial support provided
- Serve as a liaison between families and care teams to ensure coordinated, holistic care
- Participate in interdisciplinary team meetings to develop individualized care plans
- Provide crisis intervention and support for safety concerns (neglect, abuse, self-harm)
- Participate in quality improvement initiatives and continuing education
- Participate in on-call support as needed
Qualifications
- Bachelor�s degree in Social Work (BSW) required
- Master�s degree (MSW) or LCSW strongly preferred
- At least 1 year of experience in social work, preferably in hospice, home health, or a community setting
- Knowledge of end-of-life care, grief support, and cultural sensitivity
- Strong written and verbal communication skills
- Self-motivated, organized, and able to work independently
- Current Basic Life Support (CPR) certification
- Valid driver�s license, auto insurance, and reliable vehicle required
