Description
General Responsibilities
- Operate as a member of the Community Services team.
- Provide rapid re-housing intake, eligibility screening, assessment, counseling, consultation, training, crisis intervention, follow-up, and case management services.
- Manage caseload of households, providing ongoing support services including case management, goal planning, resource coordination, and advocacy to promote housing stability and self-sufficiency.
- Manage referrals through the Coordinated Entry Program for rapid re-housing in the private housing market. Provide housing-focused case management services, including budgeting, employment support, tenant education, and life skills development.
- Assist with housing location while providing housing advocacy, tenant education, and support in resolving tenant and landlord issues.
- Maintain accurate case records, documentation, and fiscal records.
- Collaborate with housing network entities to ensure safe, decent, and affordable housing.
- Conduct regular home visits and follow-up meetings to monitor housing stability and provide ongoing support as needed.
- Represent Catholic Charities in community and organizational meetings.
- Other duties as assigned within the guidelines of this position.

Requirements
- Bachelor's degree in a human services field preferred.
- Minimum of 1 year of experience required.
- Availability to work flexible days and hours.
- Capacity to conduct professional client intake and needs assessment.
- Ability to work effectively with diverse racial, cultural, family, and social backgrounds.
- Ability to work with homeless and low-income persons.
- Proficiency in Microsoft Office applications.
- Strong verbal and written communication skills.
- Passage of background clearance, valid driver's license, reliable transportation, and proof of liability insurance.
