The Case Manager/Discharge Planner is responsible for assessing patient needs, coordinating care plans, and connecting patients with appropriate medical, social, and community resources. This role works closely with physicians, nurses, and interdisciplinary teams to support patient recovery, reduce hospital readmissions, and enhance overall patient satisfaction.

The Job You Will Perform:

• Promote the mission, vision, and values of the organization
• Assess patient needs for discharge, including medical, social, and support services
• Collaborate with physicians, nurses, case managers, and social workers to create individualized discharge plans
• Arrange for home health services, rehabilitation, durable medical equipment, transportation, or community resources as needed
• Educate patients and families about discharge instructions, follow-up care, and available resources
• Document discharge planning activities in compliance with facility and regulatory standards
• Advocate for patients and assist with problem-solving barriers to discharge
• Ensure compliance with healthcare regulations, policies, and documentation standards

The Qualifications, Skills and/or Knowledge You Bring:

• Licensed Social Worker (LSW, LCSW) or Registered Nurse (RN) preferred
• Previous experience in discharge planning, case management, or a related healthcare role
• Strong knowledge of healthcare systems, community resources, insurance processes, and post-acute care options
• Strong communication, organizational, and problem-solving skills
• Ability to work collaboratively within a fast-paced healthcare environment
• Certification in Case Management (CCM or ACM) a plus
