Position Overview

The Community Transition Coach provides person-centered employment training and coaching to individuals with intellectual and developmental disabilities. This role supports individuals in developing essential job skills, improving workplace behaviors, and achieving greater independence in a variety of community-based employment and volunteer settings.


Core Responsibilities

Community Integration & Job Coaching

  • Act as a liaison between Lifetime Assistance and local business/volunteer sites.

  • Assess community sites for appropriateness and report findings.

  • Train and supervise individuals at community-based employment or volunteer locations.

  • Develop and implement individualized job instructions, accommodations, and support plans.

Vocational Development & Goal Setting

  • Assist in identifying and preparing new training or employment sites.

  • Support individuals in developing job-related behaviors, habits, and interpersonal skills.

  • Teach workplace rules, expectations, and cultural norms.

Data Collection & Documentation

  • Maintain accurate data on attendance, performance, and progress.

  • Prepare productivity evaluations per DOL regulations.

  • Submit daily, weekly, and monthly reports as required.

Program Planning & Collaboration

  • Help create Staff Action Plans aligned with each person's Life Plan and goals.

  • Attend case reviews and team meetings.

  • Recommend vocational training and transition opportunities.

Professionalism & Advocacy

  • Serve as a positive role model in appearance, behavior, and interactions.

  • Advocate for inclusive practices and respectful treatment.

  • Provide hands-on task assistance when needed.

Regulatory Compliance & Safety

  • Maintain records per agency, OPWDD, ACCES-VR, DOL, NYSID, NISH, and Medicaid standards.

  • Ensure work areas are safe, clean, and orderly.

Team Support & Training

  • Attend staff meetings, in-services, and professional development.

  • Collaborate effectively with peers, supervisors, families, and community partners.


What You Bring

Education & Experience

  • Associate degree and 1 year of related experience; OR

  • High school diploma/equivalent and 3 years of relevant pre-vocational or human services experience.

Certifications & Requirements

  • Must successfully complete all required agency trainings.

  • Must pass all required background checks (SCR, Fingerprinting, SEL, MHL).

  • Must meet agency vehicle operator requirements if applicable.

Skills & Abilities

  • Strong written and verbal communication skills.

  • Ability to read and interpret service plans, policies, and procedures.

  • Flexible, adaptive, and able to solve practical problems.

  • Organizational, critical thinking, and interpersonal skills.

Physical Demands

  • Frequent standing, walking, reaching, talking, and observing.

  • Ability to lift and move up to 40 pounds.
