Position: Licensed Clinical Social Worker
Supervised by: Chief Medical Officer
Duties and Responsibilities
● Provides clinical social work services to patients and their families.
● Counsels and provides crisis intervention for patients, including assessment and
treatment of emotional and behavioral problems.
● Coaches patients on how to manage ongoing health conditions and provides therapy to
help patients make positive behavior changes.
● Addresses referrals from primary care providers at CMC for behavioral health care
● Maintains scheduled appointments and documentation of encounters within the EMR
● Works in collaboration with a multidisciplinary team including physicians, advanced
practice providers, social workers, dieticians, nursing, and others
● Perform screenings and comprehensive assessments for mental health, substance
abuse, and other medical needs
● Provide early interventions and referrals to specialty behavioral care as needed
● Maintain client records, keeping them confidential as outlined by HIPAA
● Works with resident providers and assists in teaching them integrated behavioral health
medicine techniques
● Participates in provider meeting
● Participates in Social Work meeting as able
Qualifications
● Maintains LCSW license
● Maintains the requirements to keep a valid state license
● Submits to the necessary components to maintain privileging within the clinic including
background checks, random drug screens, peer reviews by CMO, certification of fitness
to perform duties, and willingness to comply with the various quality improvement
programs including PQRS, MU, METRIC, etc
