Description

ESSENTIAL DUTIES & RESPONSIBILITIES:

  •  Assisting with training and directing staff to follow department workflows. Team champion for workflows designated by manager, providing training and workflow oversight to care coordinators
  •  Takes active role with manager to co-lead monthly meeting sand ensure appropriate follow-up is completed, including taking minutes; support or lead weekly check-ins if manager not available.
  •  Assign/provide coverage within department in case of team members absences
  •  Support manager to maintain team training manual
  •  In person site visits to assigned PCC locations for rounding with employees, per frequency designated by manager
  •  Provides feedback on performance directly to employees as well as to manager; assists with performance evaluations
  •  Maintains equipment and supplies of team members; notifying manager and/or site Practice Manager of needed items
  •  Promotes teamwork and problem solves issues concerning patient care
  •  At the discretion of the Director or Care Coordination Manager, attends external meetings with community partners and third party payers
  •  Provides excellent customer service to internal and external customers
  •  Conducts all duties outlined in Care Coordinator job description
  •  Decrease barriers to care, increase motivation, and foster open communication
  •  Contribute to patient education materials and strategies to support care coordination
  •  Work with manager and team to create flow charts, workflows and document tracking process as needed
  •  Performs other duties as required and assigned

Qualifications


QUALIFICATIONS:

Ability to:

  • Travel between all PCC sites
  • Work at a computer terminal for extended periods of time on a daily basis

Experience/Training:

  • High school diploma required. Associates or Bachelor’s degree preferred and/or 2 years of related experience
  • Previous healthcare experience preferred
  • Demonstrated ability to effectively and efficiently handle a demanding workload involving multiple tasks
  • Demonstrated skills in the designated and certified area of practice arena and the ability to work and collaborate on a health care team.
  • EMR experience preferred

Physical Demands:

  • Must be able to remain in a stationary position 50% of the time.
  • Must be able to move around the clinic site 50% of the time.
  • Constantly operates a computer, computer printer, copy machine, and telephone.
  • Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
  • Ability to move as needed to perform necessary assessment face to face and must be able to transport from one site to another.
  • Must be able to cover other shifts as necessary.

Personal Characteristics:

  • Ability to read and write proficiently using the English language
  • Bilingual English/Spanish Preferred.
  • Excellent interpersonal skills
  • Detailed oriented with the ability to work with minimum/no supervision
  • Demonstrate flexibility to adapt to new situations
  • Self motivated and directed with the ability to prioritize and work efficiently under pressure
  • Ability to work with a team

Technical Knowledge:

  • PC, email, facsimile machine, computerized voice mail system, and common office machines
  • Ability to develop report criteria and quantitative skills to analyze basic data

Software Knowledge: 

  • Experience with MS Office applications Excel and Word required
  • Knowledge of EHR (preferably Athena) preferred
