Forensic ACT Team Clinician – Family Specialist

The Forensic ACT Team (FACT) Team Clinician – Family Specialist is a clinical member of the evidence-based team and is responsible for delivering integrated, community-based treatment and support services to individuals with serious mental illness, co-occurring substance use disorders, chronic conditions, social needs and justice system involvement. The FACT Team Clinician – Family Specialist has the lead responsibility for integrating family goals and services with the tasks of all team members and providing family psychoeducation individually and in groups. Duties include:
- Participate in daily morning meetings for the purpose of building collaborative team relationships, reviewing client progress, and engaging in comprehensive service provision.
- Deliver comprehensive, meaningful, and purposeful services in the office or field (homes, shelters, jails, community settings) as needed.
- Conduct comprehensive assessments utilizing a person-centered, trauma-informed, recovery-based approach. Develop and implement strength-based individual treatment plans that are responsive to client needs and facilitate achievement of client goals.
- Provide support, treatment, medication supervision, case management, community advocacy and linkage services, per the treatment plan.
- Provide family support and education with the goal of ensuring that family members (as defined by the client) are given the tools necessary to assist clients with success in the community.
- Develop collaborative relationships with community stakeholders including but not limited to the criminal justice system, housing providers, community treatment providers, hospital based providers and local governmental units to support treatment outcomes. Attend community meetings as assigned.
- Respond to all crisis situations and urgent needs using sound clinical judgment in order to ensure the safety and well-being of the individual.
- Provide on-call crisis intervention services on a rotating schedule amongst team members. Always act to ensure client safety, readily assess needs, provide support and interventions as needed and secure crisis interventions in urgent situations.
- Providing transportation to clients when necessary. In addition, an employee must be approved to drive agency and personal vehicles for business purposes by maintaining a valid NYS driver's license and must be approved by the agency's vehicle insurance carrier. An employee must always drive in a safe manner and use all safety devices.

Requirements
Skills/Qualifications for The FACT Team Clinician – Family Specialist include:
- Master’s degree in Social Work, Counseling, Psychology, or a related behavioral health.
- Licensure or license-eligibility (e.g., LMSW, LCSW, CASAC, LMHC) in New York.
- One to two years of experience working with individuals with a serious mental illness.
- Must have strong commitment to the rights and ability of each person with a mental illness to make their own decision and choices, live in the community, work and have access to continuous support and services to enhance wellness and recovery.
- Ability to work independently in a flexible, field-based setting.
- Experience with ACT, FACT, or intensive community-based mental health services preferred.
- Bilingual or multilingual abilities a plus.
- Knowledge of local court systems, jail diversion, and reentry services a plus.
- Valid and insurable NYS driver's license. Must have your own vehicle to use for business purposes. Agency vehicles are available for client transportation.

- $72,155-$74,152/annual equivalent, rotating on-call
