Position Summary

 

A Team Leader provides oversight for staff and is responsible for coordinating comprehensive services for individuals their program supports. The Team Leader works closely with program leadership, as well as clinical and direct support professionals to ensure that each individual is supported to live the life the way they want to live.

 

Essential Duties and Functions

Staff Supervision

  • Provides first-line supervision of direct care staff within their program through coaching and supporting their performance and professional development.
  • Organizes, schedules and distributes work among assigned personnel
  • Keeps personnel informed of pertinent policies and procedures affecting the program and/or their jobs.
  • Administers policies and procedures 

Direct Care

  • Maintain direct service case load. 
  • Coordination and oversight of medical appointments, including attending appointments if necessary
  • Communication with nurses and doctors about consumers' medical needs as appropriate.
  • Maintains positive communication and relationships with family members, guardians, staff, and Regional Office personnel about policies, issues, and/or concerns.

Administration

  • Actively participates in the person-centered planning process, including but not limited to:
    1. attending all planning meetings relevant to caseload, 
    2. providing input, 
    3. coordinating the development of training plans, 
    4. ensuring all necessary documentation is completed, submitted and reconciled, (identifies any missing documentation) 
    5. reviewing progress notes and daily notes, 
    6. monitoring for the implementation of the plan, 
  • Prepare and submit applicable reports and documentation related to individuals supported by their program. 
  • Ensure compliance with all regulatory bodies. 
  • Implement and complete continuous quality improvement activities. 
  • Other duties as assigned.

 



Qualifications

 Current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification 

  • Bachelor’s Degree in human services field, including but not limited to sociology, special education, rehabilitation, nursing, counseling and psychology OR
  • An associate degree with three (3) years’ experience working to support individuals with intellectual disabilities OR 
  • A High School Diploma with five (5) years’ experience. 
  • Valid Driver’s License

Lifting Requirements

☐  Sedentary: exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to left, carry, push, pull or move objects (including self). Most work involves sitting majority of the time.

Light: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.

☒  Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
