In this role, you’ll provide access to services and community support for individuals with mental illness and/or substance use disorders. You’ll focus on case management for homeless and chronically homeless clients in need of permanent supportive housing, while building relationships with landlords and coordinating care across systems.

Key Responsibilities:
Implement and oversee landlord relationships and lease programs
Provide case management for clients in permanent supportive housing
Assist clients in finding and retaining housing and accessing related resources
Deliver holistic, person-centered care using motivational and harm-reduction approaches
Support development of functional skills for independent living
Facilitate recovery/resiliency through goal setting and barrier reduction
Develop and update individualized treatment plans with interdisciplinary teams
Promote positive outcomes in housing, employment, education, and health
Use collaborative documentation to engage clients in treatment
Document services accurately and in alignment with treatment plans
Develop discharge and aftercare plans for continued recovery
Re-engage clients following missed appointments
Support individuals in crisis and coordinate resources
Maintain contact with hospitalized clients and facilitate discharge planning
Educate clients on managing mental illness and substance use disorders
Reinforce medication adherence and communicate concerns to prescribers
Build skills for illness self-management and wellness planning
Identify relapse risk factors and develop prevention strategies
Assist clients in accessing rehabilitative services and benefits
Coordinate with service providers, medical professionals, and community supports
Support follow-through with medical care and chronic disease management
Develop natural supports and improve interpersonal and coping skills
Provide family education and support
Help clients build skills for employment, education, and housing stability
Represent the agency professionally
Demonstrate interest in long-term and short-term goals and objectives of the company
Perform other duties as assigned by the Clinical Supervisor, Program Director, Vice President, or Executive Team member
Education and/or Experience Qualifications:
A mental health professional as defined in 9 CSR 10 – 7.140 (2) (QQ) (Missouri)
Bachelor’s degree in a human services field (e.g., social work, psychology, nursing, education, criminal justice, etc.)
Any four-year degree with two years of qualifying experience
Any four-year combination of higher education and qualifying experience
Four years of qualifying experience
Qualifying experience must include delivery of services to individuals with mental illness, substance abuse, or developmental disabilities, including:
One-on-one or group services with a rehabilitation/recovery focus
Teaching coping and management skills for psychiatric or substance use issues
Supporting employment, family, school, and community functioning
Assisting individuals in achieving goals on treatment or person-centered plans
Additional Qualifications:
Knowledge of case management and rehabilitation methods
Understanding of challenges faced by individuals with mental illness and/or substance use disorders
Familiarity with legal requirements and court procedures related to treatment
Some knowledge of supervisory and training practices in behavioral health settings
Ability to evaluate services and provide input
Strong verbal and written communication skills
Ability to produce clear and concise reports
