Your Job: 

In this high profile, fast-paced, and rewarding position, you'll collaborate with multidisciplinary team members to oversee a caseload of patients and acts as the primary liaison between the patient and the program to which they are assigned. Patient Navigator is responsible for implementing the comprehensive care plan to each patient as determined upon enrollment in the program.
 
Your Job Requirements:

•   Bachelor of Science in Nursing preferred or equivalent professional experience
•   Clinical Nursing and Case Management Experience Preferred
•   Licensed in Texas as a Registered Nurse.

 Your Job Responsibilities:

• Responsible for entry of orders and medications.

• Responsible for MA clinic staffing (assist as needed).

• Ensure all surgery patients have required testing and pre-op work completed and coordinated with the assigned provider/MA.

• Review clinic satisfaction scores and assist with back-planning to achieve key metrics.

• Responsible for nurse navigation duties; align with existing navigators to ensure cancer patients receive the complete scope of care.

• Serve as a key component in cross-pod development to ensure front and back office tasks are handled efficiently.

• Communicate clearly and openly
