As the Harm Reduction Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
- Review assessments and discharge plans provided by medical staff, CASAC, or other facility staff
- Utilize assessments and discharge planning provided by correctional facility partner to develop a plan of care
- Provide Opioid Overdose Prevention training in accordance with agency guidelines
- Provide Enrollment into the agencies Syringe Program
- Provide HIV and HCV Screening as necessary
Harm Reduction Case Manager Minimum Salary: $42,000
Harm Reduction Case Manager Maximum Salary: $42,000
Harm Reduction Case Manager Schedule: Mon – Fri 8:30am – 4:30pm
Qualifications:
- Bachelor’s Degree in health, education, and/or human services with 1 year of relevant experience
- Associate’s Degree in health, education, and/or human services with 2 years relevant experience
- At least 5 or more years in case management or care coordination with continued education towards degree in related field
