Position Summary: This member of the Aging Care and Connections Department will provide care management services to older adults, Holocaust survivor clients, adults with disabilities, and family members impacted by these populations. This position will also oversee the day-to-day operations of the Pantry Delivery Program and assist with other ACC programs as needed.


Salary Range: $24.00 - $27.00

Responsibilities

Care Management:

a. Completes initial and ongoing assessments/reassessments to identify client service and support needs.

b. Provides care management and services to Holocaust survivor clients. Assists survivor clients with correspondence from Claims Conference or Germany.

c. Provides education on and linkage to various local or state benefit programs and/or options for services that best meet the needs of the client and/or family members.

d. Provides ongoing service coordination, including but not limited to referrals, advocacy, and mediation with the client and community service providers.

e. Monitors and follows all grant requirements of the Holocaust Program. This includes but is not limited to following all guidelines for services and spending, timely and accurate reporting, and documentation. 

f. Provides ongoing service monitoring by evaluating the quality, quantity, and duration of service provided by JFS and/or external providers.

g. Occasionally provides direct assistance with transportation to appointments requiring a care manager's presence/advocacy.

h. Participates in care management office hours at off-site partner locations

i. Monitors client satisfaction with services and adjusts interventions as needed.

Establishes professional and effective communication and collaboration with    

a. clients and others involved in their care.

b. Articulates current and future service benefits, outcomes, and costs clearly to clients and/or family members.

c. Responsible and accountable for accurate and timely completion of all applicable forms, data entry, statistics tracking, and documentation of care management activities.


Pantry Delivery Program:

a. Conducts reminder calls to pantry delivery clients.

b. Assists and coordinates volunteers for delivery packing and delivery preparation.

c. Assists the Pantry Delivery program manager with data collection for Link2Feed.


Friendly Visitor Program and Senior Companion Program:

a. Assists with Friendly Visitor referrals.

b. Monitors the caseload of Friendly Visitor Clients.

c. Assists with Senior Companion program caseload.

d. Assists with Senior Companion paperwork to Spark the Change.


Additional Responsibilities:

a. Assists with other ACC program delivery as necessary, including Kosher meal delivery, the Congregate Meal Site, Pantry Delivery, Homemaker services, Counseling, and special events. 

b. Conducts program outreach and makes appropriate referrals as needed. 

c. Attends internal or external meetings, as needed, to advocate for client wishes/needs.

d. Maintains knowledge of agency-wide policies, department procedures, and funding-specific regulations.

e. Attend professional seminars, training, and in-services

f. Provides technical assistance and general support to department volunteers and student interns.

g. Provides support to agency-wide central intake as needed.

h. Actively participates in department and agency meetings as well as ad hoc committees, as requested by supervisor or director.

i. Designs and executes educational presentations to community stakeholders and JFS staff, as requested. 

j. Represents JFS at community outreach events.

k. Other duties as assigned.

Requirements

  

Education: 

  • Bachelor’s degree in social work, counseling, human services, gerontology, or related human services field preferred.


  

Experience

  • Experience and education working in social services, case management, counseling, gerontology, or social work preferred.
  • 2-3 years working with older adults and families.
  • Claims Conference experience preferred.

  

Qualifications: 

  • Effective interpersonal communication skills.
  • Knowledge of resources and available interventions for the older adult population.
  • Ability to comfort clients while conducting face-to-face assessments in their homes. 
  • Detail-oriented with strong organizational and time management skills.
  • Ability to work independently as well as in a team environment. 
  • Basic computer skills, including experience with Microsoft Office products.
  • Familiarity with client database systems. 
  • Must maintain a driver's license in good standing.
  • Must have transportation with the required level of insurance.
  • Bilingual Russian/English language skills.
