Job Title
The Registered Nurse (RN) is responsible for managing the individualized patient care by promoting and restoring patients' health through the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members. The RN with this position is also responsible for clinical projects that will help close any gaps and ensure we are HRSA compliant.
Communication
Good oral and written communication skills are required for this position. Information must be exchanged using tact and persuasion appropriately. The person in this position must be able to communicate with the patients, staff, and others; and contribute to a positive environment.
The ability to communicate in Spanish, Creole, or both is a plus.
Key Responsibilities
- Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
- Establish and maintain processes to manage scope over the project lifecycle, setting project quality and performance standards, and assessing and managing risk within, and across, multiple projects.
- Report project outcomes and/or risks to the appropriate management channels as needed—escalating issues as necessary based on project work plans.
- Manages Chronic Care Management program to include on-site employees as well as remote third-party vendors. Devise monitoring of Remote Patient Monitoring and incorporate into best practices for patient and provider to have improved outcomes in chronic conditions.
- Assures quality of care by enforcing adherence to state board of nursing and state nurse practice act requirements and to other governing agency regulations; measuring health outcomes against standards; making or recommending adjustments.
- Develop algorithms for support staff to aid and follow up on provider clinical inboxes managing patient documents to include laboratory results, consultant reports, etc., including health maintenance education and follow-up.
- Develop, implement, and regularly update clinical protocols and best practice guidelines in alignment with current evidence-based practices. Monitor and assess clinical practices to ensure adherence to established guidelines and standards.
- Provide training and mentorship to support staff, including medical assistants, LPNs, and other clinical support personnel, on clinical procedures and best practices.
- Conduct regular performance evaluations and provide constructive feedback to support staff regarding adherence to clinical guidelines.
- Performs accurate charting in medical records in accordance with WFHC policies and procedures.
- Attends educational offerings as appropriate.
- Performs other duties and responsibilities as assigned.
- May occasionally need to travel from one clinic to another for coverage.
Education & Training
- Associate or bachelor's degree in nursing
- Current RN licensure in good standing in the state of practice.
- Must be cross trained in electronic medical records, scheduling software and Microsoft Outlook.
- Understanding of standard medical practices and procedures
Experience
- One to three years of progressive work experience as a nurse in a hospital or other health care setting.
Requirements
- Level 2 background clearance required
- Proof of immunizations for MMR, HEP B vaccination, and either a history of chicken pox (varicella), or documentation of the varicella vaccine, current Tdap, and a PPD Test at time of hire.
