Job Summary
The Nurse Supervisor, Care Management is responsible for supervising the daily operations of the Care Management team while actively performing care management duties. This role combines leadership, clinical expertise, and hands-on participation to ensure the delivery of high-quality, patient-centered care and adherence to all regulatory requirements.
Essential Duties & Responsibilities
Care Management Responsibilities
- Conduct patient outreach to complete assessments, coordinate care, and connect patients with appropriate resources.
- Create and update individualized care plans for patients enrolled in care management programs.
- Improve HEDIS and preventive health screening compliance, including scheduling with specialists and vendors.
- Provide patients with educational materials to increase understanding of disease processes and required preventive screenings.
Supervisory Responsibilities
- Report to the Care Management Manager.
- Supervise nursing staff within the Care Management team.
- Lead and oversee daily operations to ensure timely and effective outreach and follow-up.
- Serve as the primary point of contact for team collaboration and support.
- Enhance staff competence by providing educational resources and balancing workload with learning opportunities.
- Approve timekeeping records and PTO requests.
- Conduct monthly chart and phone audits to maintain compliance with regulatory and organizational standards.
- Facilitate onboarding, orientation, and precepting for newly hired staff.
- Develop and implement professional development initiatives to strengthen team skills.
- Track, maintain, and report on key performance indicators (KPIs).
- Conduct annual performance reviews.
- Maintain proficiency across all lines of business, serving as a subject matter expert in care management.
- Represent care management nurses in meetings, advocacy efforts, and operational planning.
- Provide direct patient care support and coverage as needed to ensure continuity of services.
- Perform other related duties as assigned.
Education & Experience
- Bachelor’s degree in Nursing (BSN) required; four years of related work experience preferred.
- 3–5 years of direct nursing experience (RN) in a clinical setting
- Current, active RN licensure.

Physical Requirements
This position works under usual office conditions. The associate is required to work at a personal computer as well as be on the phone for extended periods of time. Must be able to stand, sit, walk and occasionally climb. The incumbent must be able to work extended and flexible hours and weekends as needed. Physical demands include ability to lift up to 50 lbs. The physical demands described here are representative of those that must be met by an associate to successfully perform the essential functions of the job.
