LPN Community Nurse | Care Resources | Mon-Fri days!
Care Resources is seeking a full-time LPN to add to our growing team of health care professionals! This position works under the supervision of the Home Care Manager and is a critical member of the interdisciplinary team.
As the LPN Community Nurse, you will:
- Perform wound care per physician orders
- Deliver and set-up medication wheels
- Adjust medication boxes or blister packs with medication changes
- Provide medication instruction
- Administer injections and immunizations per physician orders
- Perform blood draws
- Work a Monday - Friday daytime schedule with on-call every 7th or 8th weekend
Qualified candidates will have:
- Valid LPN license in good standing in the State of Michigan
- Minimum of 1 year of working with the frail or elderly population
- Prior home care experience is highly valued
- A valid driver's license and reliable transportation are necessary to make the home visits

ABOUT CARE RESOURCES:
Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) which is located in our wonderful facility at 4150 Kalamazoo Avenue SE in Grand Rapids. Care Resources provides and coordinates all medical care and social services to eligible participants. We provide all physician and nursing care, prescription drugs, durable medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, rehab, social and recreational activities at the Day Center. In order to be eligible for the PACE program, seniors must be 55 and older, a Kent County resident, able to live safely at home and meet the nursing facility level of care.
