POSITION SUMMARY

The Case Manager II will work with families and individuals to locate affordable housing units using community resources and agency networks. This position will report to the Director of Housing Services ensuring day-to-day operations.  The Case Manager II must have a flexible schedule and be adept to changing business needs.  This dynamic role demands initiative, dedication, and a genuine desire to make a positive impact on people's lives.  This position will be located at the Fairway office and will require you to travel to various locations throughout Alameda County. The Case Manager must be able to maintain a caseload while building strong relationships with clients and providing high quality care.

ESSENTIAL DUTIES

 

  • Develop and manage case plans to help families and individuals establish and achieve housing goals, including modifying goals and evaluating progress, as needed.
  • Manage a caseload of individuals and families, providing home-based case management services.
  • Provide intensive case management services for applicable households.
  • Utilize housing stability plans to assist clients in meeting housing goals, modifying goals as necessary, and evaluating progress.
  • Meet with families and individuals to assess income and expenses, determining their ability to pay rent in the future.
  • Promote and facilitate active client participation in program services.
  • Maintain accurate daily logs, case notes, and files for all clients.
  • Collect and enter program data, including HMIS.
  • Attend and participate in case review meetings, staff meetings, and training sessions.
  • Assess housing barriers for individuals and families to determine their housing and service needs.
  • Assist homeless clients in Alameda County, assessing their housing barriers and service needs.
  • Screen households for program eligibility.
  • Collaborate with assigned case managers and clients on housing searches and maintaining secure housing.
  • Promote self-sufficiency and empowerment in client interactions.
  • Provide housing and tenant counseling, housing workshops, and community resource referrals.
  • Serve as a liaison between landlords and participants.
  • Educate and advocate for clients on budgeting and financial management, including banking and credit repair.
  • Support a caseload of individuals with intensive home-based case management, collaborating 
  • with referral case managers.
  • Provide housing placement and assistance for participants, focusing on the household’s needs and goals.
  • Housing search and placement of appropriate units for potential participants.
  • Network with landlords, renter’s associations, property management businesses and county wide permanent housing providers.
  • Assist with the approval of all leases and help execute Housing Assistance Payment contracts with landlords.
  • Prepare and ensure the accurate distribution of subsidy payments to landlords as needed.
  • Provide move-in support, including perform or arrange for Housing Quality Inspections (HQI’s) prior to move-in, negotiate lease start date, review /approval of lease terms, coordinate housewarming furniture move, get documentation from landlord for check request, prepare move in documentation, upload housing documentation to Welcome Mat housing database.
  • Provide notices of contract renewals and rental adjustments.
  • Provide aid and expertise to landlords or service staff for conflict resolution and problem solving.
  • Ensure that all regulatory agreements, contracts, and fair housing laws are met as they relate to housing.
  • Be available to work Monday-Friday and occasionally during evening hours.
  • Perform other duties as assigned.

 

QUALIFICATIONS, KNOWLEDGE & SKILLS REQUIRED

  • Minimum of two years of experience working in the housing field with strong knowledge of the Alameda County housing market and geography. Must possess 1 year of field experience working with people with serious mental illness, individuals with a dual diagnosis and/or the homeless population.  
  • Knowledge of federal, state, and local fair housing laws. 
  • Prior experience using Salesforce is preferred for accurate data input and record management.
  • Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups, while providing direct services.
  • Ability to effectively intervene in crisis situations, with de-escalation techniques.
  • Proven ability to work independently and as a team, being an effective and collaborative leader. 
  • Skilled at communicating with a team responsible for their own work but sharing the responsibility of the overall success of the program. 
  • Valid California Driver’s License and access to personal automobiles.
  • Ability to work with immigrant communities a plus.
