🕒 Schedule: Part Time, 18 hours per week | 6:00 pm – 6:30 am

💰Pay Range: Pay range: $38.77 - $46.06/hr. (this rate is inclusive of the $3.40/hr. Michigan Skilled Nursing Facility Direct Care Worker Pass-Through approved rate; current approval through September 30, 2025) 

💲 Shift Differentials, Holiday, and Weekend Differentials: Beginning 11 pm – 6:59 am, $2.00 | Weekend Fri, 11 pm - Sun 10:59 pm, $0.50.

🎯 What You’ll Do in This Role:

  • In this role, the Nurse (RN/LPN) will provide comprehensive nursing care to residents, ensuring their physical, emotional, and social needs are met in a compassionate and person-centered environment. The nurse will monitor residents’ health status, administer medications and treatments, and collaborate with the interdisciplinary team to implement individualized care plans. This position also includes oversight of daily care activities performed by nursing assistants, ensuring tasks are completed in alignment with each resident’s needs and in compliance with all federal, state, and local regulations. Accurate documentation, timely response to changes in condition, and fostering a respectful, supportive atmosphere that promotes resident dignity and quality of life are all essential aspects of the role.

Primary Responsibilities:

Resident Care and Support

  • Deliver direct nursing care, including administering medications, performing treatments, and monitoring vital signs.
  • Conduct thorough health assessments and observe residents for changes in condition, reporting concerns to the appropriate clinical team members.
  • Provide wound care and other specialized nursing procedures as needed.
  • Monitor residents for adverse reactions or side effects and adjust care plans accordingly.

Collaboration and Guidance

  • Collaborate with interdisciplinary teams to coordinate care and enhance residents' quality of life.
  • Oversee and support nursing assistants and other care staff, offering guidance and mentorship as appropriate.

Documentation and Reporting

  • Complete daily and monthly documentation that accurately reflects the care provided and residents’ responses to interventions.
  • Perform admission and discharge procedures as assigned.
  • Maintain accurate and up-to-date medical records in accordance with facility policies and regulatory requirements.

Compliance and Safety

  • Adhere to all safety, infection control, and regulatory protocols to maintain a safe environment for residents and staff.
  • Follow proper infection prevention practices to reduce the spread of illness.
  • Participate in ongoing education and training to maintain licensure and stay current on best practices in nursing care.
  • Uphold professional standards, including resident confidentiality, ethical conduct, and respect for resident rights.

 What You’ll Need:

  • Bachelor’s degree in nursing (BSN) preferred; an associate degree may be considered with relevant experience.
  • Current Registered Nurse (RN) 
  • Current CPR or Basic Life Support (BLS) certification or willing to obtain upon hire.
  • Previous experience working with the elderly or in geriatric care preferred.
  • Prior experience in a skilled nursing or long-term care environment strongly preferred.
  • Ability to lift and move objects weighing up to 35 pounds without assistance.
