Job Description:

This position is responsible for providing clerical and organizational support to the assigned area/department.  Works under general supervision in providing pharmaceutical care for ambulatory patients, provides pharmaceutical clinical opinion and input to providers and clinical staff to enhance or maintain quality patient care. PRIMARY JOB DUTIES:

By referral, may independently see patients under a Collaborative Practice Agreement to provide pharmaceutical care.

Reviews medication profiles for appropriate drug selection, dose, route and frequency. 

Provides education to providers and nurses about new medications.

Works with patient navigators, prior authorization specialists, and billing/coding personnel to evaluate cost-effectiveness and accessibility for medications.

Serves as a resource to help with patient assistance program enrollment.

Identifies and resolves actual drug-related problems and develops plan to prevent potential drug-related problems.

Improves systems and processes to ensure safe, accurate and timely provision of all aspects of pharmaceutical care.

Guides pharmacists through educational programs to increase knowledge of appropriate pharmacotherapy use and patient monitoring.

Serves as a resource to the outreach sites where our providers see patients.

Performs other duties as reasonably required or assigned.
