SUMMARY
This position is responsible for assessing, planning, implementing, monitoring, and evaluating actions required to meet the consumer’s needs.

JOB DUTIES
- Assess consumer needs based on their needs, limitations, and desires.
- Develop a case plan with the consumer.
- Maintain constant communication with the consumer.
- Maintain consumer’s confidence and protect operations by keeping information confidential.
- Coordinate and provides services that are client centered.
- Provide trauma-informed, culturally competent holistic services to victims.
- Develop and maintain client case records
. - Manage case assignments, draft service plans, review case progress and determine case closure.
- Collaborate with consumers and appropriate community agencies to coordinate and facilitate services for consumers.
- Assist consumers in making informed decisions by acting as their advocate.
- Attend court proceedings, if necessary.
- Actively participates in team review of records and cases.
- Follow policies and procedures, participate in quality reviews, reporting needed changes.
- Provide information to the public regarding the program.
- Provide case management/file documentation for individual caseload and submits biweekly and quarterly reports in a timely manner.
- Provide outreach and support services, i.e., mobile advocacy.
- Develop professional relationships with other Federal, State, and Tribal agencies, in order to develop resources for consumers.
- Perform other duties as assigned.
NECESSARY KNOWLEDGE, SKILLS, AND ABILITY
- Knowledge of health, social services, legal issues, and other general/local services throughout the Fairbanks area.
- Ability to learn and apply program/agency operating policies and procedures.
- Ability to report to work in a timely manner.
- Ability to operate computers utilizing a variety of computer software and ability to type. Ability to complete digital and paper forms accurately.
- Understanding and sensitivity to diverse cultures and lifestyles, and ability to work effectively with Alaska Native populations.
- Skill in managing multiple priorities and tasks concurrently and meet deadlines.
- Skill in oral and written communication.
- Skill in establishing and maintaining cooperative and respectful working relationships with children, parents, supervisors, and other employees.
- Ability to work independently as well as with teams.
- Ability to effectively present instruction, and/or information to individuals, staff, administrators, and the public.
MINIMUM QUALIFICATIONS (Education & Experience)
- Bachelor’s degree in social work
- Minimum two (2) years of work experience in case management OR
- Minimum two (2) years of direct service to the targeted population or similar experience
- Strong understanding and sensitivity to diverse cultures and lifestyles, with demonstrated experience working effectively with Alaska Native population.
PREFERRED QUALIFICATIONS (Education & Experience)
- Master’s degree in social work
