JOB SUMMARY:
This position provides rotating support to the Medical Respite Programs at the RISE Center and Sojourner House. The role serves as the Case Manager providing initial intake, ongoing case management, basic support and ensuring the safety of clients during their stay in the programs. This position will also possibly provide evening coverage, as needed.
JOB RESPONSIBILITIES:
- Provide operating support to the program manager as needed. Report any maintenance issues at the facility to the program manager.
- Provide case management services including advocacy, crisis intervention and counseling to program participants.
- Complete initial intake and necessary documentation for all respite clients.
- Orient new clients to the program and settling them into their unit as needed.
- Assist clients to acclimate and adjust to residing in the setting and in understanding program rules.
- Complete case notes, services all HMIS and Salesforce data necessary and contribute to staff debriefings. Review case notes and clinical notes with staff and care partners as needed.
- Develop individual service plan with participant and review plan quarterly for ongoing needs and adjust accordingly.
- Provide wellness checks on all residents daily and address basic needs, and work with client on housing goals and opportunities as the client recovers.
- Work together with the clinical teams to assure that client is attending all medical appointments (this may include case manager transporting client) as required to maintain health.
- Work with Coordinated Entry and the housing staff to facilitate the transition to the next level of housing and assist with move-out and acclimation process.
- Mitigate and manage any on-site crisis as they arise.
- Report and document all incidents, emergencies, substantial behavioral difficulties, and any intrusions or visits by police, ambulance, or any other outside agencies to the supervisor or on-call manager immediately.
- Assist clients in following program rules and policies using trauma-informed care and harm reduction techniques.
- Be aware of the impact of trauma on participants and prevent and/or respond quickly and compassionately to triggers and post-trauma responses.
- Work in partnership with other staff, interns and staff from other agencies and medical centers.
- Participate in training as assigned, maintain certification in CPI (Non-violent Crisis Intervention) and other techniques of working with clients according to the schedule of trainings provided by the program director, the agency, your supervisor, and the Coordinated Care team.
Requirements
QUALIFICATIONS:
- Bachelor’s degree in social service or high school diploma with three years of experience.
- Good communication, strong interpersonal and listening skills.
- Office, Salesforce and HMIS.
- Bi-lingual (English/Spanish) a plus.
- Ability to build and maintain rapport with clients.
KEY COMPETENCIES:
- Ability to follow policies and procedures.
- Must have smartphone to utilize Agency communication and collaboration tools.
- Must participate in training.
- Must have own transportation, valid driver’s license and proof of insurance.

SALARY: $48,000 - $50,000
