Role and Responsibilities 


Position Competencies: 

  1. Interview patients to obtain medical information and measure their vital signs, weight, and height 
  2. Medication reconciliation 
  3. Log and document to industry/insurance standards  
  4. Regular 360 Care 

Position Responsibilities: 

Specific: 

  1. Coordinating and assisting in developing patient-specific health plans of care 
  2. Identifying health and social determinants of health in the assigned population 
  3. Actively working with care team, both internal and external, to meet patient needs 
  4. Monitoring key metrics of the assigned population to ensure goals are met 
  5. Communicating between the care team, patients, and family/caregivers 
  6. Educating patients about their condition(s), medication(s)  
  7. Making patients’ appointments with providers to ensure care plan adherence 
  8. Maintaining patient confidentiality and helping to ensure quality care 
  9. Other projects/duties as assigned potentially patient care in the clinics as needed.

Qualifications and Education Requirements 

  1. Education: High School Diploma with Subsequent completion of Medical Assistant or Certified Nursing Assistant curriculum, college degree preferred. A combination of education and experience may be accepted in lieu of education 
  2. Experience: Two years demonstrated experience in a medical office or health plan care management 
  3. Certification/Licensure: Valid Florida Driver License (unencumbered and no points); Certified Medical Assistant (within six months of hire if not current at time of hire) or CNA 
  4. Training and Knowledge Required at Time of Hire: Excellent computer skills with experience in Outlook, MS Office Suite, EMRs; Strong presentation skills, positive and effective communication skills; Good typing skills with quality documentation accuracy  
  5. Age Specific Individuals Served/Responsibility: Adults 
  6. Security Measures and Clearance: may require – up to a Level 2 Background approval 
  7. Strong presentation skills, positive and effective communication skills 
  8. Typing at least 70 words per minute with 100% accuracy
