ECM Case Manager

The ECM Case Manager is responsible for outreach, engagement, and providing support and linkage to wellness/recovery resources both in person and virtually to assist clients in reaching the optimal level of wellness and functional capability.  The ECM Case manager works with highest risk Medi-Cal members with the most complex medical and social needs, connecting with all their providers to coordinate areas of care and support.

Duties and responsibilities

  • Conducts outreach to engage and enroll eligible community members to the Cal AIM ECM program.
  • Completes high-risk behavioral health and housing assessments with client to identify need areas.
  • Develops and maintains an individualized care plan for each client.
  • Coordinates care for clients with a multidisciplinary clinical team and outside agencies.
  • Facilitates referral linkages, navigation, performs case management services, and supports securing community resources.
  • Assists clients with the development of social and community support systems.
  • Develops effective working relationships with agencies, organizations, and providers to advocate for clients.
  • Documents and maintains clinical records per appropriate standards and regulations.
  • Conducts outreach in diverse community settings including shelters, drop-in facilities, and residential settings.
  • Participates in clinical supervision, consultations, and training.
  • Provides translation and language support services as needed.
  • Completes other duties and related projects as assigned.

Qualifications


Education & Experience:

  • Bachelor’s degree in healthcare or behavioral health related field, or equivalent training and experience.
  • One year or more of case management or related experience.

Knowledge, Skills, & Abilities:

  • Familiarity with community-based resources including housing.
  • Excellent problem-solving and organizational skills.
  • Understanding of cultural issues, community resources and family dynamics.
  • This position may require driving. A valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability policies.
  • Trauma informed training preferred.
  • Verbal and written fluency in English required; Bilingual skills in a second language (particularly Spanish, Mandarin, Vietnamese) desired but not required.

  • Ability to communicate well with people of diverse cultural professional and experiential background
  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.

Working conditions

This job requires you to be in the community, in client’s homes, and other community partner sites. Work is often conducted in an office environment. Basic safety precautions and the use of protective clothing or gear may be required. Driving is essential to the function of this job.
