Duties

Serve as the installation Primary Care Behavioral Health (PCBH) program lead providing oversight of the PCBH program.,Screen and assess patients and perform appropriate biopsychosocial evaluations.,Conduct brief assessments and formulate evidence-based interventions appropriate to a primary care setting for a wide range of behavioral health conditions, chronic medical conditions, and health behaviors affecting medical conditions.,Assist with implementation and monitoring of treatment plans with Primary Care Managers (PCMs) and other primary care team members.,Provide concise documentation of care and recommendations in patient’s medical record.

Basic Requirement for Social Worker (Primary Care Behavioral Health): Degree: Master's degree (or higher degree) in social work from a school accredited by the Council on Social Work Education. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes conducting bio-psychosocial evaluations of patients, providing individual or group clinical counseling, and developing treatment plans or recommendations. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (NH-02).Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted. AND Licensure: This position requires a current, active, valid, and unrestricted clinical license to practice Social Work independently offered by a U.S. State, District of Columbia, Commonwealth, territory, or jurisdiction.
