Wood River Health is committed to providing team-based, patient-centered care in alignment with the Patient-Centered Medical Home (PCMH) model. We are seeking a dedicated Quality Engagement Nurse/Risk Manager combines clinical nursing responsibilities with quality improvement initiatives. This position supports data collection, patient outreach, and internal audits, while helping maintain the Organization’s PCMH accreditation and compliance with quality standards. The Quality Engagement Nurse/Risk Manager reports to the Chief Medical Officer and collaborates closely with the Quality Improvement Coordinator.

ESSENTIAL DUTIES; include but are not limited to:

  • Track and document quality measures in the Electronic Health Record (EHR).
  • Conduct patient outreach to address care gaps and promote preventive services.
  • Assist with audits and reporting for UDS, HEDIS, NCQA, RIDOH, ACOs, and other quality programs.
  • Learn and successfully submit for our annual NCQA PCMH re-attestation, with assistance from our Chief Medical Officer, Quality Improvement Coordinator, and analysts.
  • Participate in Plan-Do-Study-Act (PDSA) quality improvement projects.
  • Attend Quality Management Committee meetings and professional development sessions with the intent of becoming expert in NCQA/PCMH, UDS and other reporting and quality methodologies.
  • Maintain strict confidentiality in all matters
  • Facilitate the development, implementation, and review of the WRH Quality Improvement Plan and the Safety and Risk Management Plan Assuring WRH Board of Directors approval every three years or if significant changes are made prior.
  • Participates in the implementation and oversight of the Corporate Compliance Program 
  • Develops and implements a clinic risk management program, including identification of categories and types of risk
  • Recommends to senior leadership and board committees for all categories and types of risk, the levels of risk to be retained by the health center versus that which should be protected against through the acquisition of insurance
  • Completes an Annual Risk Management Report on the status of the risk management program to the WRH Board of Directors and appropriate management. Assuring that Quarterly Risk Assessments are conducted and appropriate follow up is conducted
  • Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information 
  • Enliven and support the mission, vision, and values of Wood River Health
  • Adhere to organizational policies and procedures and Wood River Health Compliance Program Standards
  • Performing other duties as assigned to meet business needs


EDUCATION AND EXPERIENCE: 

  • Bachelor of Science in Nursing, or related healthcare field          
  • Three plus years of experience in nursing, compliance, and/or risk management
  • Additional and ongoing risk management training 
  • Electronic Medical Record experience, preferred  
  • Experience working in a community health center, preferred  
  • Ability to effectively use MS Office Suite, Internet, and email
  • Excellent interpersonal skills and ability to work effectively at an independent level and with others in the workplace
  • Foster a culture of quality improvement among peers and support staff.
  • Live the mantra that the patient is never to blame, we must remove the barriers.
  • Cultural sensitivity necessary to work with a diverse patient and staff population

LICENSURE AND CERTIFICATION:

  • Full, unrestricted, Registered Nurse (RN) license in the State of Rhode Island 
  • CPR Certification, required
