Description
We are currently seeking a compassionate and organized Case Manager to join our team. This is a full-time position responsible for overseeing the total wellbeing of a designated caseload of residents, in accordance with New York State Enriched Housing regulatory requirements.
The Case Manager serves as a key liaison between residents, families, healthcare providers, and internal teams, ensuring individualized care plans are developed, implemented, and monitored effectively. This role plays a critical part in supporting residents’ independence and quality of life.
Requirements
- Manage and monitor a caseload of residents to ensure all care and service needs are met.
- Conduct assessments and develop individualized service plans based on resident needs and preferences.
- Maintain detailed documentation and ensure compliance with NYS Enriched Housing regulations.
- Coordinate services with medical professionals, community resources, and families.
- Participate in interdisciplinary meetings and care planning.
- Respond to urgent resident needs and participate in on-call rotation (including occasional weekends).
- Serve as an advocate for residents’ rights and wellbeing.
- Required: Valid New York State LPN License or a degree in Social Work from an accredited program.
- Experience in senior living, long-term care, or community health preferred.
- Strong communication, organizational, and problem-solving skills.
- Ability to manage multiple priorities and work collaboratively in a team environment.
- Familiarity with electronic medical records and documentation systems is a plus.
