Title:    Licensed Vocational Nurse

Department:    Adult Day Services

Reports To:    Director 

Summary:     Under the direction and supervision of the Registered Nurse, provide direct nursing care and maximize the comfort and health of Adult Day Services participants.  

Essential Functions:

  Provides skilled nursing care to all participants. 

  Assists in implementing nursing care plans, including but not limited to, checking participants’ body weight, vital signs, O2 saturation, blood glucose level and administering tube feeding; provides follow-up as needed.

  Monitors participants’ changes in condition and reports to the Registered Nurse and/or family if indicated. 

  Administers and dispenses all medications and maintains appropriate medication records. 

  Maintains appropriate nursing records according to the centers’ policies and procedures.

  Disposes of medications, needles, syringes, and hazardous waste materials according to the program’s procedures.

  Assists in evaluating participants’ needs (s) for evaluation and/or services by a registered dietitian, physical therapist, occupational therapist, speech therapist, podiatrist, and/or other medical services.

  Provides counseling to participants and/or caregivers on issues related to medical and/or personal care as needed, including self-medication training.

  Acts as liaison to participants’ primary care physicians.

  Maintains an inventory of medical supplies and equipment, medications ordered by the participants’ personal care physicians, and PRN medications.

  Assists in overseeing the provision of personal care services by Program Aides.

  Participates in morning huddles, staff meetings, Individual Plan of Care (IPC) meetings and other meetings as assigned.

  Checks designated equipment such as glucometer and oxygen cylinders daily.

  Provides health education sessions in consultation with the Registered Nurse if necessary. 

  Performs other duties as assigned.


Qualifications:

  Current valid California LVN license. 

  Previous nursing experience working with the elderly or the chronically ill, or experience in home health, public health, or acute care preferred.

  Good interpersonal skills with the ability to work independently and as part of a multidisciplinary team.

  Excellent written and oral communication skills in English and Cantonese.

  Familiar with different patient transferring techniques. Must pass a strength test for physical fitness for the job prior to employment and every 2 years thereafter: Ability to transfer participants of up to 120 lbs, stand, walk, bend/twist/turn the neck/waist, reach, grip/grasp, pinch/squeeze, exercise fine manipulation of the hands and wrist movements, kneel, squat, and push/pull up to 75% of time. 

  Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.

  Must acquire CPR and First Aid certificates within 6 months of employment and maintain current certificates thereafter.
