Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.
Program Description: The Senate is a permanent supportive housing residence for formerly homeless adults, many with mental illness and substance abuse. On-site services include medication monitoring, case management, medical care, entitlement assistance, substance abuse counseling, money management, 24 hour/7 day per week front desk security, and daily group activities.
Position Summary: The Registered Nurse is responsible for delivering direct and comprehensive nursing care for Senate tenants. The Registered Nurse will support the coordination of medical care and psychiatric care to increase positive health outcomes for tenants.
Schedule: Monday-Friday 9:00am-5:00pm
Salary Range: $99,000-$111,000
Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.
Roles, Responsibilities, and Essential Duties
Role, Responsibilities, and Essential Duties:
- Administer medications, injections, and treatments.
- Monitor, pack and dispense medications to clients.
- Keep record of client medications and medications schedules in client charts.
- Document client care services by charting in client records and supporting data tracking and analysis.
- Provide wound care.
- Collaborate with on-site Nurse Practitioners, community physicians, psychiatrists and case managers to ensure optimal treatment plan for clients which includes assisting with setting up onsite clinics, ensuring attendance, and conducting sign out with both medical and behavioral health providers.
- Coordination of care with client providers and local pharmacy.
- Provide education to clients regarding chronic illness management and prevention.
- Assess acutely ill clients and determine appropriateness of emergency services.
- Conduct home visits to provide in-home care for clients who are home-bound and as needed.
- Communicate with social service staff any medication or health changes
- Track and maintain medical supply inventory.
- Conduct biweekly wellness groups
- Attend staff meetings, case conferences, training programs and supervision meetings
- Other duties as required to maintain medical and behavioral health of clients.
- Other duties as required in accordance with agency policies and funding source requirements.
Qualifications/Educational Requirements:
- NYS RN license required.
- BSN preferred.
- Spanish speaking preferred.
Skills, Knowledge and Abilities
- Knowledge of working with older adults, adults with mental illness and substance use concerns.
- Understanding of harm reduction and trauma informed care.
- Ability to multitask in a fast-paced environment.
- Strong verbal and written communication skills.
Computer Skills Needed
- Ability to utilize electronic health record databases.
- Ability to work in Microsoft Office Suite, proficient in Microsoft Outlook, Excel, and Teams.
- Able to use or learn to use AWARDS.
Physical Requirements
- Sitting for an extended period of time standing and walking
- Visit clients in their apartments in The Senate Residence
- Able to lift up to 15 lbs.
Work Environment
- Mostly indoor office, occasional basement setting during group activities and events.
- Regular home visits to client apartments required.
