What will you do as a Case Manager, Family Centered Therapy?

As a Case Manager, Family Centered Therapy in the Residential department, you will experience working with children and teens who have acute mental health/behavioral challenges is necessary. Family Centered Treatment is an evidence-based model that requires you to become certified within one year.

As a Case Manager, Family Centered Therapy you must be able to complete clinical documentation, including treatment plans and crisis plans. As the FCT practitioner, you will be responsible for meeting with the caregivers at least once per week for three hours to complete FCT interventions to prepare them for reunification with the child. Once the child returns home, you will meet with the entire family twice a week for a total of four hours.

A typical day as an a Case Manager, Family Centered Therapy will include meetings with families, discussing cases with residential FCT practitioners, and completing documentation. There is an on-call rotation for after-hours, weekends, and holidays. There will be a virtual team meeting on Monday afternoon and one hour of virtual individual supervision during the week. A one-hour skills lab occurs virtually on Fridays. Otherwise, you create your schedule based on the family’s availability. Most families prefer afternoon and evening appointments as that is when children are out of school.

As a community-based practitioner, you will need to be available on some weekends to complete sessions when the youth is home for a visit. This is an in-person service when possible. You are reimbursed for gas mileage.

Requirements

Minimum Qualifications/Requirements:

• Education requirement for this individual contributor role is: Bachelor’s degree.
• A minimum of 2 years of relevant professional experience and lived experience
• Proficient in Microsoft Office Suite application software
• Excellent written and oral communication skills
• All potential job candidates must pass a drug screening test, and an extensive background check is required.
• Must have a valid North Carolina Driver License and meet any credentialing, licensing and privileging standards as it pertains to the department you are in.
