Systems Navigator
We are seeking a Systems Navigator to join our Behavioral Health Home team! This program utilizes a multi-disciplinary team to deliver person-centered services designed to support a person in coordinating care and services while reaching their health and wellness goals. It is team-based case management working with clients who are diagnosed with chronic mental illness. The program coordinates care and integrates social services, mental health services and primary care, to support health and wellness goals. The Systems Navigator acts as the central point of contact for clients and ensure that they get their behavioral and physical health needs met in a coordinated way.
Hours: Full-Time, Monday-Friday, 8am-4:30pm
Location: New Hope Clinic - 5555 Boone Ave. N., New Hope, MN 55428.
Hiring Range: $22.00-$23.75/ Hourly, based on qualifications
Hiring Bonus: $1,000 Hiring bonus to external candidates

Main Job Duties:
- Must have physical presence in the Twin Cities, Minnesota area
- Coordinating care between behavioral and physical health specialists
- Educating clients regarding possible resources and supporting clients in accessing those services
- Maintain accurate and timely case notes on all client contacts and document all activities tied to the client. Ensure compliance with Behavioral Health Home standards as set forth through Minnesota’s Department of Human Services.
- Assessing client needs to support the creation of individual development plans
- Collaborating with a multi-disciplinary mental health team
- Promoting recovery, stability and self-sufficiency
Requirements
Required Qualifications:
- Minimum age of 21 years
- Ability to pass all background checks
- Excellent driving record, reliable car, and current auto insurance
AND EITHER
- Master’s degree in a behavioral science or related field; OR
- Bachelor’s degree in a behavioral science or related field and proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; OR
- Has completed 30 semester hours or 45 quarter hours in behavioral science or a related field and:
- Has proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; OR
- Has 2,000 hours of experience in the delivery of services to people with mental illness; OR
- Has 2,000 hours of experience in the delivery of services to people with traumatic brain injury or developmental disabilities and completes training on mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects; OR
- Is fluent in the non-English language of the cultural group to which 50% of the practitioner’s clients belong;
OR
- Has a high school diploma and 4,000 hours of experience in the delivery of services to people with:
- Mental illness; OR
- Traumatic brain injury or developmental disabilities and completes training on mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects;
OR
- Has a high school diploma and 2,000 hours of experience in the delivery of services to people with:
- Mental illness, emotional disturbance, or substance use disorder, and receives clinical supervision from a mental health professional once a week until 4,000 hours of experience is met; OR
- Traumatic brain injury or developmental disabilities and completes training on mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects and receives clinical supervision from a mental health professional once a week until 4,000 hours of supervised experience is met; OR
- Is currently enrolled in a graduate level behavioral sciences program at an accredited college or university and is formally assigned to the program for clinical training;
- Must be free of chemical use problems for at least two years immediately preceding their hiring and must sign a statement attesting to that fact, if applicable.
OR
- Bachelor’s Degree in the behavioral sciences or related field (i.e. social work, psychology, or nursing from an accredited college or university), and 2,000 hours of supervised experience in the delivery of services to people with serious and persistent mental illness.
