Primary Purpose

Develop and lead the planning and implementation of community health outreach strategies through delivery of care, education and support that address specific disparities and barriers to care across all the communities served by Atrium Health Floyd.

Major Responsibilities

  • Collaborate with other departments to create and enact service line initiatives that position Atrium Health Floyd as the community’s health expert.
  • Cultivate relationships with collaborative partners in the community. Educate teammates about community outreach efforts and responsibilities.
  • Research and pursue outside funding opportunities for Atrium Health Floyd initiatives, including grants and congressional directed spending opportunities. Write grant requests and manages application process.
  • Provide education for all hospital boards and community benefit subcommittees about funding opportunities and community health engagement activities.
  • Help implement enterprise Social Impact strategies.
  • Prepare an annual budget for community health outreach activities.
  • Maintain a calendar of community health efforts.
  • Schedule regular outreach initiatives and educate teammates on community responsibility.
  • Prepare accurate records and report on the goals of initiatives.
  • Coordinate and prepare required Community Benefit Report and Community Health Needs Assessment for Atrium Health Floyd Medical Center, Atrium Health Floyd Polk Medical Center and Atrium Health Floyd Cherokee Medical Center.
  • Serve on Atrium Health Floyd’s Diversity, Equity and Inclusion Council.
  • Represent Atrium Health Floyd on various civic and nonprofit boards throughout its service area.
  • Share corporate messages by speaking to area business, civic and faith organizations.
  • Help implement enterprise Acute Social Needs strategies, including oversite of the Community Resource Hub.

Minimum Job Requirements

Education

Bachelor’s Degree in a related area of study or equivalent experience

Certification / License

Work Experience

A minimum of 5 years of experience specializing in community outreach including the development of strategy through implementation

Knowledge / Skills / Abilities

A thorough understanding of community health and DEI principles, practices, and procedures. Experience in health and education is a plus.

Physical Requirements and Working Conditions

Ability to work weekends and evenings as needed.

Preferred Job Requirements

Education

Master’s degree in business administration, healthcare, public health, nonprofit or related field.

Pay Range

$53.80 - $80.70
