POSITION SUMMARY

As a Physical Therapist, you will complete comprehensive patient assessments, develop a patient-

centered plan of care and implement best practice physical therapy interventions to help patients

achieve their highest desired state of health and well-being.

ESSENTIAL JOB FUNCTIONS

1. Conduct comprehensive assessment of patients' history, systems review, and tests and measures

to identify impairments, functional limitations and disabilities and to establish a physical therapy

diagnosis, a prognosis for recovery and appropriate physical therapy plan of care.

2. Implement purposeful and skilled therapeutic interventions and modalities to address patient

deficits and maximize their functional mobility.

3. Collaborate with interdisciplinary teams including providers, nurses, physical therapists, speech

therapists, and social workers to optimize patient care and outcomes.

4. Maintain accurate and timely documentation in accordance with organizational and department

policies and regulatory standards.

5. Keep informed regarding advancements in physical therapy practice and participate in

professional development activities to enhance clinical skills and knowledge.
