As a Care Coordination Liaison, your responsibility is to deliver an exceptional experience to patients throughout the respiratory referral process as part of their discharge process. In this role, you are also responsible for order intake and providing equipment education to customers, family members and other caregivers. All your efforts are essential to our ability to continue comprehensive, world-class care.
A caregiver in this position works days from 8:30AM – 5:00PM, with one monthly Saturday requirements.
A caregiver who excels in this role will:
- Identify appropriate referral sources within the assigned hospital(s) / business units and build and maintain positive relationships.
- Serve as the primary contact for referral sources.
- Provide education to the referral source and patients regarding Home Respiratory Therapy services and equipment (e.g., oxygen systems and nebulizers).
- Analyze referral source data, monitor the local market and the competition and identify growth opportunities within assigned facilities and departments.
- Recommend marketing initiatives based on trend analysis and enterprise/department strategy, working with the marketing team to develop collateral.
- Assess, process and streamline respiratory referrals for service initiation.
- Ensure timely and adequate documentation and assure prescriptive adherence of all referrals.
- Educate on DMEPOS order requirements and competitive bidding.
- Coordinate and monitor proper equipment inventory to accommodate starts of care volumes.
- Procure all necessary equipment documentation during instruction at start of care.
- Maintain compliance with all regulatory and accrediting bodies.
- Maintain necessary CE requirements as set forth by regulatory agencies.
Minimum qualifications for the ideal future caregiver include:
- High School Diploma/GED
- Knowledge of medical diagnosis and medications terminology
- Two years of related healthcare experience, including independent decision making and the ability to provide communicate with patients in a hospital setting
Preferred qualifications for the ideal future caregiver include:
- HME experience

Physical Requirements:
- Ability to lift a minimum of 40 lbs
- Ability to properly move oxygen cylinders
- Manual dexterity to operate office equipment
- May require periods of sitting, standing and the ability to walk to various locations.
- Ability to travel regionally.
- Must have normal or corrected vision.
