GENERAL SUMMARY:

The Therapist works within an Outpatient Substance Abuse & Mental Health Clinic. They provide group, individual, and in-home mental health services to clients ages 13 and up (including adults) under the supervision of the Director of Behavioral Health. They are required to meet all expectations as noted and outlined in DHS Chapters 35, 36 & 75.

ESSENTIAL FUNCTIONS:

  • Provides individual outpatient, and/or in-home/telehealth when appropriate and approved.
  • Provides group counseling when appropriate and desired.
  • Completes assessment, treatment plans and discharge summaries for/with each client.
  • Completes progress notes and summary reports to document the direct services provided.
  • Submits billing documentation, as required by referral and funding sources, in the agency Electronic Health Record (EHR) system.
  • Collaborate with referral sources and other professionals as appropriate (ex: CCS external billing EHR).
  • Participate in clinical consultation and monthly team meetings as appropriate.
  • Adheres to St. Charles Youth & Family Services Policies and Procedures.
  • Attend trainings and meetings as required.
  • Facilitate group sessions when appropriate.
  • Other duties as assigned.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

  • Master’s degree in Social Work or related field strongly preferred.
  • Licensed by the State of WI as an LPC, LCSW, or LMFT or related licensure required.
  • Experience with SUD and dually diagnosed clients preferred.
  • Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms and values the worth of individuals, families and communities and protects and preserves the dignity of each.
  • Knowledge of DSM equivalent version at time of hire as used within the field at the time.
  • Strong written and verbal skills.
  • Knowledge of state statutes related to social work.
  • Valid Wisconsin driver’s license and vehicle liability insurance.
  • Ability to use vehicle for work related duties according to the agency’s insurance liability standards.
  • Ability to communicate effectively with clients and other staff.

WORKING CONDITIONS:

  • Indoor environment to include in-office, telehealth, in-home, and community settings.
  • Local travel as required fulfilling essential functions of the position.
  • Flexible schedule, some evenings required based on program and/or patient needs.
